r
points are accessible, they shall be inspected to ascertain whether pollution control measures are
effective in preventing significant impacts to receiving waters. Where discharge locations are
inaccessible, nearby downstream locations shall be inspected to the extent that such inspections
are practicable.
5.4 Modifying the SWMP Based on Results of Inspections
Based on the results of the inspection, the SWMP shall be modified as necessary to include
additional or modified BMPs designed to correct problems identified. Revisions to the SWMP
shall be completed within seven (7)calendar days following the inspection or identification of a
condition requiring attention to resolve a pollution condition. If existing BMPs need to be modified
or if additional BMPs are necessary, implementation shall be completed as soon as practicable.
5.5 Inspection Report
A report summarizing the scope of the inspection, name(s) and qualifications of personnel making
the inspection,the date(s) of the inspection, and major observations relating to the
implementation of the SWMP shall be made and retained as part of the SWMP for at least three
(3)years after the date of the inspection. Major observations should include:
• Location(s)of discharges of pollutants from the site;
• Location(s)of BMPs that need to be maintained;
• Location(s) of BMPs that failed to operate as designed or proved inadequate for a
particular location; and
• Location(s)where additional BMPs are needed that did not exist at the time of inspection.
Such reports shall identify any incidents of noncompliance with the SWMP and the Permit.
5.6 Non-Storm Water Discharges
Except for flows from fire-fighting activities, sources of non-storm water listed in Part I.C.2. of the
Permit that are combined with storm water discharges associated with quarrying activity must be
identified in the SWMP. Unexpected non-storm water related runoff will be identified and added to
this SWMP in the following space:
Equipment maintenance and wash waters
Non-stormwater discharges will be inspected and monitored and the quality of such discharge will
be identified and described in an inspection report.
14
