Exhibit D-Mine Plan Amendment 01 <br /> In 2013 the May Day No. 1 portal was stabilized in accordance with TR-1 and used as the <br /> primary access to the underground mine workings. <br /> The Chief Manway will be used as an emergency escape way and for mine ventilation(Refer to <br /> Exhibit D-Figure D-10). The manway was reconstructed in 2016 in accordance with TR 5, TR 7 <br /> and TR 8.When necessary, an amendment or TR will be submitted if a surface disturbance <br /> activity outside of the affected mine area is anticipated. <br /> Other affected mine areas include the Idaho mine area (See Figure D-11) and pre-existing roads <br /> (See Figure 12 a and 12 b). <br /> When necessary, blasting will be done in accordance with the protocols described in the <br /> Geotechnical Stability Exhibit. 6.5.4 and will comply with Mine Safety and Health <br /> Administration(MSHA)and the Bureau of Alcohol, Tobacco and Firearms Administration <br /> (ATF)requirement. <br /> Mining and underground exploration activities work will be performed in accordance with <br /> MSHA regulations. <br /> The following workers will be engaged in mining activities: <br /> • two to three contract workers will be involved in blasting activities; <br /> • six to eight workers will conduct exploration and ore production activities; <br /> • two to four workers will be involved in ore removal and transport of ore from the mine <br /> site to a third party contracted custom mill. <br /> Power to the May Day No.1 work area will be provided by 110kW diesel powered generators <br /> located at the May Day No. 1 portal. Diesel fuel for May Day Idaho mine complex will be <br /> stored and handled in accordance with the Environmental Protection Plan(Exhibit U, Section <br /> 6.4.20.7)and MSHA regulations <br /> Phase 3-Production Expansion <br /> Based on the results of the underground exploration activities,the underground operational <br /> area may be expanded. If surface disturbance outside of the existing affected permit area is <br /> required, a DRMS permit amendment will be submitted for approval. <br /> Phase 4- Mine Completion and Closure <br /> At the end of the life of the mine,disturbed areas will be reclaimed in accordance with <br /> the approved Reclamation Plan(Exhibit E). Closure of the mine will include demolition <br /> of the mine buildings and structures and removal of salvageable material from the permit <br /> area.Non-salvageable materials will be buried in place or in the adits and stopes; or, if <br /> warranted removed from the permit area and disposed offsite in accordance with all <br /> applicable laws, rules, or ordinances. <br /> 112(d)Permit <br /> May Day and Idaho Mine and Mill Application <br /> M-1981-185 <br /> Revised-03.08.2017 <br /> Exhibit D—Rule 6.4.4 Page 5 of 13 <br />
