"Technical Revision No. 1,As-built report,Access Portal Collapse Repair and Equipment Work Plan, May <br /> Day No. 1 Portal." <br /> On December 15, 2016,the Board approved the reclamation work. <br /> Requirement No.4 <br /> The Operator may not commence mining and milling activities until the Division receives and <br /> approves, through the Technical Revision or Amendment process as determined by the Division, <br /> adequate information for ambient surface and ground water quality for the Division to verify hydrologic <br /> baseline conditions, and a hydrologic monitoring plan has been fully approved by the Division and <br /> implemented by the Operator as necessary for the division to verify compliance with water quality <br /> regulations and the Act and Rules. The Operator has commenced the required monitoring of surface <br /> water and has proposed the locations for installation of four initial ground water monitoring wells. The <br /> water quality issue may require coordination with other agencies, including but not limited to, the USFS. <br /> Response No.4 <br /> Exhibit G "Water Resources" (attached) has been updated to address the hydrologic baseline <br /> conditions including appropriate water quality data. The Operator will continue routine water sampling <br /> in accordance with the approved permit. During the development of Exhibit G,the Operator <br /> collaborated with the Colorado Division of Water Resources (DWR) and the Colorado Department of <br /> Public Health and the Environment(CDPHE)to address their potential concerns. Water quality <br /> characterization, monitoring, and reporting required under Rule 6.4.21(8)(9)and (11)for a Designated <br /> Mining Operation has been addressed in the attached Exhibit. <br /> Requirement No. 5 <br /> The Operator may not commence mining and milling activities until the Division receives and <br /> approves, through the Technical Revision or Amendment process as determined by the Division, mining <br /> and reclamation plans, with associated work plans, which satisfy the requirements of Rules 6.4.4, 6.4.5, <br /> 6.4.6, and 6.5. <br /> Response No.5 <br /> Rule 6.4.4 Mining Plan—The Operator's mining plan is presented in Exhibit D "Mining Plan" <br /> (attached). <br /> Rule 6.4.5 Reclamation Plan—The Operator's reclamation plan is presented in Exhibit E <br /> "Reclamation Plan" (attached). <br /> Rule 6.4.6 Reclamation Plan Map—The Operator's reclamation plan maps are presented in <br /> Exhibit F "Reclamation Plan Maps" (attached). <br /> Rule 6.5 Geotechnical Stability Exhibit—Geotechnical Stability is addressed in Exhibit 6.5 <br /> "Geotechnical Stability—Blasting" (attached). <br /> March 10,2017 <br /> Amendment to Permit No. M-1981-185 <br /> May Day Idaho Mine Complex <br />
