Requirement No.6
The Operator may not commence mining and milling activities until the Division receives and
approves, through the Technical Revision or Amendment process as determined by the Division, design
specifications certified by a licensed professional engineer for all Environmental Protection Facilities, as
required by the Act and Rules, and the Operator satisfies the specific requirements of Rules 7.3 and 7.4.
The Environmental Protection Facilities include, but are not limited to, the following: mill building;
storage sheds and tanks in which designated chemicals are stored or utilized, tailings facilities;and
augmentation pond and its associated pipeline and discharge structure.
Response No.6
On April 1,2013 the Division approved the Technical Revision request(TR-6)addressing the
removal of the mill from the operations plan.
Exhibit U "Environmental Protection Plan" (attached)addresses adopted environmental protection
methods and procedures for the mine site.
Requirement No.7
The Operator may not commence mining and milling activities until the Division receives and
approves, through the Technical Revision or Amendment process, an updated Exhibit L and financial
warranty addressing all applicable requirements of Rule 4, and specifically, but not limited to, Rule
4.2.1(4)regarding reclamation costs associated with the protection of water resources, including costs to
cover necessary water quality protection, treatment and monitoring as required by permit, and the Act
and Rules. The existing Exhibit L,submitted by the Operator does not address this issue and is not
adequate for approval.
Response No.7
Exhibit L"Reclamation Costs" (attached)summarizes anticipated reclamation costs. The
Operator has received a Colorado Department of Public Health and Environment (CDPHE) industrial
Discharge Permit(COR 031-1717). The CDPHE permit stipulates water quality discharge monitoring and
reporting requirements. The discharge point(Idaho Springs) currently complies with the applicable
water quality standards. (See Exhibit G for water quality data.)
Respectfully Submitted,
Sara Glinatsis
Sunrise Mining,LLC
Manager
March 10,2017
Amendment to Permit No. M-1981-185
May Day Idaho Mine Complex
