c. Reactive aggregate test reports. <br />d. Ratio of fine to total aggregates. <br />e. Weight (saturated surface dry) of each aggregate per cubic yard. <br />f. Total water content in gallons per cubic yard and proposed source. <br />g. Slump on which design is based. <br />h. Brand, type, and quantity of cement. <br />i. Brand, type, and quantity of admixtures. <br />j. Air content. <br />k. Two (2) sets of compression test cylinders (two [2] cylinders per set) shall be <br />made for each proposed mix. <br />1) Test one (1) set of two (2) cylinders at age seven (7) days and other set at <br />twenty-eight (28) days. <br />1. Statement of intended use for mix. <br />m. Submit one copy of a representative concrete batch trip ticket. <br />n. Signature of qualified mix designer. <br />D. Water holding basin repair methods: As necessary if water -holding basins fail tests as <br />specified. Leaking cracks shall be repaired by the epoxy injection process. <br />E. Cold weather curing and protection: Detailed plan for cold weather curing and protection <br />of concrete to be placed and/or cured in weather below 40 degrees Fahrenheit (40°F). <br />Include requirements and recommendations of ACI 306 and ACI 318 for minimum seven <br />(7) days. <br />F. Patching procedures: As necessary if defects occur in the as -cast and cured concrete to <br />obtain finish specified. <br />G. Cylinder Compression Test Reports. <br />1. Submit two (2) copies of certified test reports to Engineer for air content and concrete <br />compression testing. <br />H. Ready -mix Delivery Tickets. <br />1. Submit delivery tickets for each load at the time of delivery indicating the following: <br />a. Quantity delivered, batch plant name, and batch time. <br />b. Mix design used including design strength. <br />c. Batch weights. <br />d. Amount of initial water added. <br />e.
