Squaw Gulch VLF CQA NewFields Job No.475.0106.006 Week Ending Saturday,July 16,2016 Soil Liner Fill (SLF) placement was performed in the areas shown on Figure 2. Subgrade was approved and moisture conditioned (when required) prior to SLF placement. SLF material was hauled to the Phase 1 VLF from local stockpiles using CAT 740 haul trucks. The material was then spread by low ground pressure CAT dozers equipped with GPS. Moisture conditioning of SLF material was performed in place using a tandem-axel water truck, and compaction was achieved using a 10-ton vibratory smooth drum compactor. Moisture content verification, nuclear density testing, and depth verification were performed, with results meeting the required project technical specifications, prior to accepting the surface for geomembrane installation. All test locations were surveyed by Ames. 1.6 Drain Cover Fill (DCF) Drain Cover Fill (DCF) placement was performed in the areas shown on Figure 4. CAT 740 haul trucks hauled DCF from the stockpile to the Phase 1 VLF. The DCF was placed in a 2-foot lift on previously approved geomembrane by low ground pressure CAT dozers. Haul routes consisted of 4-foot lifts that will be spread into 2-foot lifts at a later date. Ames' laborers were present during placement to assist in removing debris/sandbags, installing high volume solution collection piping, and ensuring that the lift thickness was correct. A NewFields' representative constantly monitored the DCF placement. 1.7 Riprap No work performed during this reporting period. 2.0 GEOSYNTHETICS 2.1 Geogrid No work performed during this reporting period. 2.2 Geomembrane Installation Geomembrane installation continued within the Phase 1 SGVLF in the areas shown on Figure 3. Approximately 195,053 square feet (P-1980 to P-2044) of 80 mil LLDPE (double sided texture) was deployed. Subgrade was inspected and approved by NewFields and ECA representatives prior to geomembrane deployment. Approximately 9,030 linear feet of seam was fusion welded using three fusion welding machine/operator combinations. Twenty destructive testing samples were marked during fusion seaming. Approximately 1,136 linear feet of seam was extrusion welded using one extrusion welding machine/operator combination. Five destructive testing samples were marked during extrusion seaming. All destructive tests performed met project specifications. Non-destructive testing was performed for fusion welded seams. Repairs Page 3
