Newmont Mining Corporation 0 0 <br /> Cripple Creek&Victor Gold Mine . <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,July 16,2016 <br /> 1.0 EARTHWORKS <br /> 1.1 Underground Working Remediation <br /> Concrete plugs were poured for remediation of UG workings #6444 (one plug), #6477 <br /> (three plugs), and #6478 (one plug). A total of 184 cubic yards of concrete was placed in <br /> the plugs in accordance with project technical specifications. A total of 5 sets of <br /> concrete cylinders were cast for compressive strength testing. Slump, air entrainment, <br /> and temperature testing was also performed for each sample. All field properties test <br /> results met project specifications. Further remediation is required. <br /> Blasting was performed at the Upper Anaconda Mine Complex (AMC). Two shots were <br /> performed collapsing the crown pillar in each area. An additional shot will be performed <br /> in Monday July, 18, 2016 and investigation of the Upper AMC will begin by excavation. <br /> 1.2 Structural Fill <br /> Structural Fill placement continued within the Squaw Gulch VLF Phase 1 footprint. NewFields <br /> personnel inspected fill material and observed placement and compaction procedures to <br /> ensure that they were in accordance with the project technical specifications. Ambient <br /> temperatures remained above 32 degrees Fahrenheit throughout the shift. The following <br /> structural fill construction activities occurred during the shift: <br /> Slope contouring (cut to fill) in areas bounded by bench stations: <br /> o 1-9+00 to 1-11+00 and the Haul Road <br /> y Compaction was achieved using a smooth drum roller. <br /> 1.3 Subgrade <br /> No subgrade acceptance was performed during this reporting period. Figure 1 shows the <br /> previous subgrade acceptance areas. <br /> 1.4 Leak Detection Trench <br /> Approximately 150 feet of leak detection trench were constructed along the High Grade Mill <br /> slope in accordance with the project technical specifications. <br /> 1.5 Soil Liner Fill (SLF) <br /> Soil Liner Fill processing continued at the Dump 1 processing plant. Approximately 184,699 dry <br /> tons of material has been processed as of Saturday,July 16, 2016. <br /> Page 2 <br />
