King I topsoil stockpile located above the refuse pile <br />Once favorable weather for reseeding occurs and seeding takes place, these items will be considered <br />resolved and subsequently monitored for seeding success. <br />2. Errosion at culvert crossing the road at Reach 10 of the southern drainage ditch at King I had been <br />armored with large blocks of sandstone. Item resolved, see (photos 12 and 13 ). <br />3. The line of survey stakes marking the perimeter of the #2 stack tube need to be remarked with bright <br />pink tape. Item resolved. <br />4. Gully forming behind water storage tank requires mitigation within the month, photo 1; as does the <br />ditch along the road below water storage tank and pump house. Item resolved. <br />5. Culvert C18 requires clean out. Item resolved. <br />6. Equipment slid into drainage ditch along the road to the belt portal. Item resolved. <br />7. Reset waddle and rocks at substation and yard drop structures to prevent incursion of debris, (photos <br />9A, B, C.). Item resolved. <br />8. Culvert C20 requires reestablishment of drainage regime within the month, (photo 5B). Item <br />resolved. <br /> <br />NEW MAINTENANCE ITEMS <br />June 2016 <br /> <br />A. The ditch along outcrop (reach 10, King I access toad) had been graded over and requires <br />reestablishing (photo 14). <br />B. Monitor and proactively treat weeds such as white top, cheat grass, mullin and thistle, especially on <br />topsoil piles, (photos 4, 10). <br />C. Assure that surface flow reaches containment, see photo 6. <br /> <br /> <br />INSPECTION: <br />SIGNS AND MARKERS Rule 4.02 <br />o The mine site identification signs inside the entrances to the King I and King facilities II from <br />County Road 120 were intact, displaying all required information: mine name, operator name, <br />address, phone number as well as DRMS permit number and contact information. <br />o As stated in the permit; boundary markers actually indicate the surface disturbance area. Markers <br />identified during this inspection were installed as required throughout the areas visited and appeared <br />in good repair. <br />o Topsoil stockpiles observed were clearly marked with identifying signs. <br /> <br />ROADS Rule 4.03 <br />Construction 4.03.1(3), 4.03.2(3); Drainage 4.03.1(4), 4.03.2(4); Surfacing and Maintenance 4.03.1(5) and <br />(6) 4.03.2(5) and (6).
