Newmont Mining Corporation •■ <br /> Cripple Creek&Victor Gold Mine ■ <br /> Squaw Gulch VLF CQA <br /> NewFields Job No.475.0106.006 <br /> Week Ending Saturday,May 07,2016 <br /> 1.3 Subgrade <br /> Subgrade was accepted and approved for geogrid installation at the fifth step of the lower <br /> Anaconda Mine Complex. <br /> An excavator was used to prepare subgrade for riprap installation at the Phase II Diversion <br /> Channel. <br /> 1.4 Leak Detection Trench <br /> No work performed during the reporting period. <br /> 1.5 Soil Liner Fill (SLF) <br /> SLF processing continued at the Dump 1 processing plant. Approximately 125,212 dry tons of <br /> material has been processed as of Saturday, May 7, 2016. <br /> 1.6 Drain Cover Fill (DCF) <br /> Drain Cover Fill was placed using a low ground pressure dozer. An existing DCF haul route on <br /> the 9650 foot bench was pushed up the slope in the area below and between stations HM6+00 <br /> to HM7+00. The attached figure shows the area where fill was placed during this reporting <br /> period. <br /> 2.0 GEOSYNTHETICS <br /> 2.1 Geogrid <br /> Approximately 7840 square feet of geogrid (total) was placed for the second and third layer at <br /> UG#6679/UG#6471. Approximately 16,110 square feet of geogrid (total) was placed for the first <br /> and second layer at the fifth step of the lower Anaconda Mine Complex. <br /> 2.2 Geomembrane <br /> ECA performed repairs on previously installed, non-certified geomembrane within Phase 1 <br /> along the H bench. Ames worked to clean the geomembrane of all deleterious material prior to <br /> ECA performing repairs. Nondestructive testing was completed on all repairs. <br /> The geomembrane that is being repaired was placed at the end of 2015, and was left exposed <br /> to potential damage over the winter. All damages are on geomembrane that was previously <br /> approved for DCF placement. No new defects were marked; however, a NewFields <br /> Page 3 <br />
