table contains 34 wells, whose locations are mapped on Map 10B. Several questions relate to
the table:
a. The response is sufficient.

d. The table contains 11 wells named DH... none of which are shown on the map. Please
clarify.
CCC responded that the DHH series of wells are shown on Map 1113, which shows all drill
hole locations. Rule 2.04.7(4) requires "A map prepared according to the standards of
2.10 showing... (b) Elevations and locations of monitoring stations used to gather data
for water quality and quantity..." In the Colowyo PAP Map 1013 is given the title
HYDROLOGIC FEATURES and MONITORING LOCATIONS, and should show all current and
former monitoring wells.
Please update Map 10B to show the locations of all wells in Table 2.04.7-38.
Please also update the text in Volume 15, Rule 2, Page 21. (Incidentally, the currently
proposed text refers to Map 11C, which is presumably an error)
CCC updated Map 10B to show the locations of all wells listed in Table 2.04.7-38, with
the exception of DHSS -78-1, whose location is not precisely known. Explanatory
footnotes were added to Table 2.04.7-38, and the text in Volume 15, Rule 2, Page 22
was updated.
The response is sufficient for the purposes of this revision, (however CCC should
attempt to locate and map DHSS -78-1, since it will need to be properly plugged and
abandoned prior to bond release).
75. Completion information for the monitoring wells was not readily located.
Please refer, in the text or as a footnote to table 2.04.7-38, to sources for well completion
information, including the screened interval, for all monitoring wells. (For example: Exhibit 26,
Item 1 in Volume 20 has completion information for 5 of the 34 wells described in table 2.04.7-
38).
Monitoring well completion information should be consolidated and readily located in the PAP.
Exhibit 7, Item 21, Appendix 5A provides completion information for The MWC-04 sequence of
wells. Appendix 5B provides completion information for five of the valley fill wells: MS -04-01,
MS -04-02, MC -04-01, MC -04-02, and MLC -04-01. The proposed footnote to table 2.04.7-38
should include a reference to completion information for the remaining wells in the table, to be
found within the PAP.
This item has not yet been adequately addressed.
