4 (FORM 2 -Public filing) <br /> 11. Describe proposed surface excavation or other land disturbance,including roads,pits,trenches,waste piles,drill pads and <br /> collar areas of underground workings,ponds,etc... <br /> Access to underground workings will be through existing adit;Surface excavations limited to:1)on-site disposal of sludge material from <br /> existing settling bold. Material has been lebted for rnetalb toxicity and acid base potential(bee attached report),material will be mixed <br /> with lime for neutralization and placed in a trench excavated in an abandoned roadway which is clear of drainage ways,and covered with <br /> the native material borrowed trom the excavated trencTi. I he trench will be lined and covered with PVC or LUPE liner to prevent any <br /> degradation of the background environment;and,2)possible excavation as part of rehabilitation of passive water treatment system in <br /> area of old bio-reactor passive treatment system installed as part of past CDPHE 319 project(see Exhibit N,Part IV,Section 1). <br /> 12. Proposed Disturbance(approximate)Describe the proposed drilling to be conducted,including anticipated number of holes, <br /> diameter,depth,location,etc... Hand channel sampling(250 samples)will be conducted on exposed structures to verify <br /> existing resource Informanbn; Umbering of old underground workings and ground control work wlil be conducted as necessary to <br /> ensure safety of personnel conducting work. If grouting is determined to be a probable,viable method of shutting off the source <br /> of underground water,then a grouting plan will be developed and submitted to the Division for review. <br /> F. Other Disturbances(please describe): <br /> No other disturbances are anticipated at this time other than repair of existing structures <br /> and general site clean-up of past mining operation debris except as described in Part III, <br /> Section 11. <br /> G. Indicate Chemicals and Fuels used or stored on site. List type,quantity and method to store. <br /> Diesel Fuel-500 gallon fuel storage tank with weather protection and full secondary containment vessel under fuel tank,will <br /> supply portable generator,portable air compressor&mobile equipment. Tank will be re-supplied by fuel cell mounted on a <br /> pickup truck containing less than 100 gallons of diesel fuel. Lubricants for equipment will be stored in a covered area and <br /> provided with secondary containment. Grouting materials,if necessary for grouting program,will be determined at the time <br /> F. If so,please describe: <br /> The project will utilize existing site roads. No new roads will be constructed. Limited <br /> Manintenance will be conducted on existing roads.
