PERMIT #: M-1980-149 <br />INSPECTOR’S INITIALS: PSH <br />INSPECTION DATE: February 12, 2016 <br /> <br /> <br />Page 2 of 8 <br />perimeter drain. <br />CORRECTIVE ACTIONS: The Operator shall provide as‐built drawings for the perimeter drain along the western <br />side of Phase 2 by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 5/09/16 <br /> <br />INSPECTION TOPIC: Availability Of Records <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Operator committed to install a perimeter drain along the <br />southern edge of the site, along the existing alignment of Firestone Blvd. The Division was not notified the <br />perimeter drain was installed prior to the installation of the clay liner and slurry walls and has not received as‐ <br />built drawings for the installed perimeter drain. <br />CORRECTIVE ACTIONS: The Operator shall provide as‐built drawings for the perimeter drain along the southern <br />edge of the site by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 5/09/16 <br /> <br />INSPECTION TOPIC: Availability Of Records <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Operator committed to supply copies of the agreements with <br />the gas line companies for the slurry wall crossing with the gas lines in the Phases 3 and 4 during construction of <br />the slurry walls. The Division did not received copies of the agreements with the gas line companies during the <br />construction of the Phases 3 and 4 slurry walls. <br />CORRECTIVE ACTIONS: The Operator shall provide copies of the crossing agreements with the gas line <br />companies in Phases 3 and 4 by the corrective action date. <br />CORRECTIVE ACTION DUE DATE: 5/09/16 <br /> <br /> <br />OBSERVATIONS <br /> <br />The Hokestra Pit was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br />(Division/DRMS) as part of the Division’s monitoring inspection program. The site was previously inspected by <br />the Division on March 9, 2012 for acreage release no. 5 (AR‐05). Ms. Valerie Mino with Weld County was <br />present during the inspection. The site access was limited due to the wet ground conditions at the site. <br /> <br />The site is permitted for 131.60 acres and is divided into four (4) remaining phases; Phase 2, Phase 3, Phase 4 <br />and the parcel east of Phase 3 containing 2 ponds. <br /> <br />Phase 2 in located in the northwest corner of the site and consists of a clay lined reservoir. The Operator has <br />not submitted the Office of the State Engineer (SEO) approval for the clay liner to the Division. The upland <br />areas around the reservoir will be seeded this spring according to the Operator. The Phase is in the process of <br />being sold for water storage to the City of Firestone. The Phase was dry and the north slope of the Phase was <br />reinforced with riprap. The riprap is required as part of the flood mitigation measures for the site. The <br />Division did not observed the river bank reinforcement adjacent to Phase 2. The required spillways between <br />Phase 2 and 4 and Phase 4 and the St. Vrain River have not been constructed. <br /> <br />Phase 3 is located in the southeast corner of the site. The Phase was recently lined with a slurry wall. The <br />Operator has not submitted the SEO approval for the slurry wall to the Division. The location of the slurry wall <br />centerline was marked on all sides with white posts. The Phase has been stripped of topsoil and overburden. <br />Mining excavation has not started in the Phase. The ponds to the east of the Phase will need to be drained <br />before Phase 3 is mined, since the two areas are surrounded by a shared slurry wall.
