The Phase was recently lined with a slurry wall. The Operator has not submitted the SEO approval for the slurry wall to the Division. The location of the slurry wall centerline was marked on all sides with white posts. The Phase was actively mining and dewatering. The mining excavation is progressing from the north to south. A processing plant and stockpile storage area was observed in the southern portion of the Phase. The parcel east of Phase 3 was mined and reclaimed to developed water resource years ago. The Operator has not submitted the SEO approval for the slurry wall to the Division. The Division will require proof of SEO approval for the slurry wall prior to consideration of future release requests. The parcel slopes and vegetation could not be evaluated due to limited access of the area. As cited as problems, the Operator committed to the following items during the amendment No. 3 application process which have not been completed according to the Division’s files: The Operator committed to providing baseline groundwater monitoring plan data to the Division prior to the installation of the liners (clay and slurry walls). The Division has not received the baseline data to date. The Operator shall provide the baseline groundwater monitoring data for the Division’s file. The Operator committed to submit the monthly monitoring data to the Division on a quarterly basis following the submittal of the initial baseline groundwater data. The Division has not received the quarterly groundwater data. The Operator shall provide all available groundwater monitoring data for the Division’s file and commit to provide the monthly monitoring data with the annual report starting with the 2017 Annual Report. The Operator committed to install a perimeter drain along the western side of the site, adjacent to the clay liner in Phase 2. The Division was not notified the perimeter drain was installed prior to the installation of the clay liner and has not received as‐built drawings for the installed perimeter drain. The Operator shall provide as‐built drawings for the perimeter drain along the western side of Phase 2. The Operator committed to install a perimeter drain along the southern edge of the site, along the existing alignment of Firestone Blvd. The Division was not notified the perimeter drain was installed prior to the installation of the clay liner and slurry walls and has not received as‐built drawings for the installed perimeter drain. The Operator shall provide as‐built drawings for the perimeter drain along the southern edge of the site. The Operator committed to supply copies of the agreements with the gas line companies for the slurry wall crossing with the gas lines in the Phases 3 and 4 during construction of the slurry walls. The Division did not received copies of the agreements with the gas line companies during the construction of the Phases 3 and 4 slurry walls. The Operator shall provide copies of the crossing agreements with the gas line companies in Phases 3 and 4. The Division will require the Operator to verify the required 4H:1V slopes of the reservoirs by submitting survey data verifying the grading prior to Division consideration of any future final or partial release requests at the site, if the reservoir are filled with water at the time of the release inspection.
