OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br /> <br />INSPECTION TOPIC: Availability Of Records  <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Operator committed to providing baseline groundwater  <br />monitoring plan data to the Division prior to the installation of the liners (clay and slurry walls).  The Division has  <br />not received the baseline data.    <br />CORRECTIVE ACTIONS: The Operator shall provide the baseline groundwater monitoring data for the Division’s  <br />file by the corrective action date.  <br />CORRECTIVE ACTION DUE DATE: 5/09/16  <br />  <br />INSPECTION TOPIC: Availability Of Records  <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Operator committed to submit the monthly monitoring data to  <br />the Division on a quarterly basis following the submittal of the initial baseline groundwater data.  The Division  <br />has not received the quarterly groundwater data.  <br />CORRECTIVE ACTIONS: The Operator shall provide all available groundwater monitoring data for the Division’s  <br />file by the corrective action date and commit to provide the monthly monitoring data with the annual report  <br />starting with the 2017 Annual Report.  <br />CORRECTIVE ACTION DUE DATE: 5/09/16  <br />  <br />INSPECTION TOPIC: Availability Of Records  <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Operator committed to install a perimeter drain along the  <br />western side of the site, adjacent to the clay liner in Phase 2.  The Division was not notified the perimeter drain  <br />was installed prior to the installation of the clay liner and has not received as‐built drawings for the installed
