CRIPPLE CREEK & VICTOR GOLD MINING Co. <br />ANGLOGOLD ASHANTI (COLORADO) amec <br />SQUAW GULCH (VLF) WEEKLY REPORT foster <br />wheeler <br />A technical memo will be generated to address deviation of design with respect to the <br />tertiary underdrain. <br />Leak Detection: <br />A Cat mini -excavator prepared approximately 200 LF of leak detection trench. The leak detection <br />was completed per project plans and specifications between stations FFO+00 and FF2+00. <br />Miscellaneous: <br />Amec personnel continued LVSCS pump monitoring and observation. <br />B) Underground Workings: <br />UG #6050: A Cat excavator placed Structural Fill in preparation for geogrid subgrade. Further <br />remediation required. <br />UG #6004: John Deere and Cat excavators removed shot rock from the working in preparation <br />for geogrid subgrade. Further remediation required. <br />UG #6455: Continued drilling operations, 2,100LF. Further remediation required. <br />UG #6456: Continued drilling operations, 450LF. Further remediation required. <br />UG #6673: Production drilling and a production blast occurred during this reporting period. <br />Further remediation required. <br />UG #6004: Removal of shot rock to prepare geogrid subgrade. <br />C) Geomembrane: <br />Geomembrane installation within the PSSA continued this week performing deployment, <br />seaming, repair, and QA/QC activities. Geomembrane installation was performed at the ADR <br />pad and PSSA northwest slope. Approximately 174,511 square feet of 80mil LLDPE <br />geomembrane was deployed. Deployment includes panels installed to replace portions of <br />removed 2014 geomembrane (P103 — P114). Soil Liner Fill was inspected and approved by <br />AMEC, Ames and ECA representatives prior to geomembrane deployment. Approximately 7,787 <br />linear feet of seam was fusion welded during deployment using one or two fusion welding <br />machines for each day of deployment. Approximately 704 linear feet of seam was extrusion <br />welded during deployment using one welding machine for each day of deployment. Destructive <br />and non-destructive testing was completed per technical specifications. <br />Ames continuously excavated anchor trench ahead of geomembrane installation. After <br />geomembrane testing was completed, the anchor trench above finished panels was backfilled <br />and compacted per project specifications. <br />Panels P96, P98 and P201 - P212 (80mil LLDPE) were accepted by AMEC, Ames and ECA <br />representatives during this reporting period. <br />See attached figure for approved geomembrane. <br />II) Storm Water Management <br />Best Management Practices (BMPs) continued to be performed. <br />
