Permit M1990-057 <br />Leadville Mill, Union Milling Company, LLC <br />June 17, 2015 <br />6. Signage. Permanent mill permit identification signs (metal -style) were maintained on the <br />main gate to the facility and on the mill building, identifying the permitee and permit <br />number. <br />7. Fencing. The barbed wire property boundary fence was repaired in various areas where <br />it was broken by wildlife. Fence maintenance is required by our Lake County Conditional <br />Use Permit. <br />8. Permit Boundary. Permit boundary stakes were maintained as required. <br />9. Mill Facility Activity. The mill buildings and equipment were maintained in preparation of <br />reconditioning and restarting the mill. <br />10. Sediment Control. Sediment control measures were performed on an as needed basis <br />(temporary silt fence construction around the ore stockpiles, temporary historic tailings <br />storage, and new tailings storage facility). Additionally, culverts were cleaned, and berms <br />and roads were maintained as required. <br />11. Ore Stockpiles. Ore stockpiles remain covered with visqueen material as a temporary <br />measure to minimize acid generating potential of sulfidic ores. This visqueen material <br />remains in place as long as the material remains in place. Repairs were made as <br />necessary on visqueen covers in early -June 2014. <br />12. Weed Control. Weed control measures were performed as required and according to the <br />weed management plan. Identified weed species are Oxeye daisies and Canada thistle. <br />Weeds occur sporadically near access roads, the mill building and the fence bordering <br />the water treatment plant (East side of property). Over the last 4 years, weeds have been <br />significantly eliminated. Current practice involves walking around property and spraying <br />weeds with Milestone monthly (5 to 7 oz per acre application rate) June - September. <br />13. Permitting Activity. The following permitting activities were performed during the year. <br />• UMC submitted a status report to the Lake County Commissioners as required by <br />our Conditional Use Permit (CUP. The commissioners informed us that for 2014 <br />and in the future submission of a letter report was sufficient and that presenting <br />our activities in a public forum was no longer required <br />• An amendment to M1990-057 to convert from a 110(2) to a 110(d) was approved <br />by CDRMS on June 11, 2014. UMC supplemented the reclamation bond by <br />$12,730, bring the total to $64,430. <br />• On May 1, 2015, UMC received notice from CDRMS that Lockland LLC has <br />received approval of a Hard Rock 110(2) Reclamation Permit and that UMC can <br />request release of financial warranty associated with this property. UMC are <br />addressing this request and are in the process of submitting a written Notice of <br />Completion and Request to Release Financial Warranty for a DMO. <br />• UMC continues to maintain stormwater and APENs with CDPHE. <br />14. Reclamation/Bond. No permanent reclamation occurred at the mill in the last 12 month <br />period. Additional interim stabilization activities continued within existing disturbed areas. <br />The bond for both the Leadville Mill and Hopemore Shaft totaling $94,430 remained in <br />place. The $81,700 amount was determined by the Division in November 2007 and an <br />additional $12,730 was added in June 2014 with respect to the DMO at the Leadville Mill. <br />The Leadville Mill reclamation estimate is $64,430 and the remaining $30,000 is for the <br />Hopemore Shaft. An Irrevocable Letter of Credit remains in place for $64,430 from the <br />Bank of Colorado of Yuma, CO for the Leadville Mill bond. <br />
