Permit M1990-057 <br />Leadville Mill, Union Milling Company, LLC <br />June 17, 2015 <br />MW -2, Down -Gradient Well MW -3, Up -Gradient Well <br />Water samples were collected and tested for 3 consecutive quarters since the completion <br />of MW -2 and MW -3. Q4 2014 samples were collected on 16 December 2014 and <br />submitted under chain of custody to Wyoming Analytical Laboratories (WAL) for analysis. <br />As this was the first sample collected, UMC commissioned Mr. Andrew Harley, <br />Geochemist at Tetra Tech Engineering to provide proper guidance for water sample <br />collection. Results and recommendations by Mr. Harley were prepared in a letter report <br />and submitted to CDRMS on 20 January 2015. <br />Q1 2015, and Q2 2015 samples were collected on 27 March 2015 and 30 May 2015, <br />respectively and submitted to Test America Labs for analysis.
