Permit M1990-057 2 <br />Leadville Mill, Union Milling Company, LLC <br />June 17, 2015 <br />Annual Report <br />The 110d Annual Report Form submitted along with our Annual Fee Invoice and CDRMS Report <br />Request dated 1 May 2015 is completed, signed and included with this documentation. See Page <br />7. <br />Property <br />Permit M-1990-057 includes the Leadville Mill. The Hopemore Shaft has submitted an application <br />for a separate permit. The permit operator is: <br />Union Milling Company, LLC (UMC) <br />Nick Michael, Member <br />P. O. Box 620490 <br />Littleton, CO 80162-0490 <br />303-947-3499 <br />The Leadville Mill is located in the Northeast Quarter of the Northeast Quarter of Section 33, and <br />the Southeast Quarter of the Southeast Quarter of Section 28, Township 9 South, Range 80 West. <br />The county designated address is 13815 Highway 24, Lake County, Colorado. The owner of the <br />Leadville Mill is: <br />Constructive Investments, LLC <br />Nick Michael, Member <br />PO Box 620490 <br />Littleton, CO 80162-0490 <br />Activities: June 18, 2014 to June 17, 2015 <br />The mill remained idle, on care and maintenance status, with no production or milling occurring <br />in the last 12 month period. Care and maintenance were the only activities that occurred during <br />the past year at the mill. <br />The Leadville Mill activities for the past 12 months are presented below. <br />1. Groundwater Quality Sampling. UMC completed 2 new monitoring wells (MW -2 and MW - <br />3) on 24 October, 2014. <br />On 28 May 2014 UMC submitted a proposal ("Leadville Mill, M1990-057, Proposed <br />Monitoring Well Locations") to CDRMS. The proposal was to construct Monitoring Well <br />(MW) No. 2 as the down -gradient well at the extreme southwest corner of the property, <br />and MW -3 as the up -gradient well northeast of the TSF near our lower mill access road. <br />MW -2 is at approximate coordinates N39013'44.17", W106°19'59.98"", elevation 9701ft, <br />and MW -3 is at approximate coordinates N39013'49.87", W106019'53.61", elevation <br />9744ft (assuming coordinates as reported by Google Earth). Locations are shown on our <br />updated Exhibit provided with this report. <br />UMC received a verbal "OK" accepting the proposal for the well locations; however no <br />formal letter was received by UMC. <br />UMC, applied and obtained permits from the State Engineer (Permit 295655 for MW -2, <br />and 295654 for MW -3) on 11 September 2014. We then secured a drilling contractor <br />who completed the wells on 14 October 2014. UMC then completed the monitoring well <br />project on 24 October 2014 by installing well pumps. <br />Photos of MW -2 and MW -3 are provided below. <br />
