REPORT OF UNDESIRABLE EVENT (05/11/15) <br />DATE/TIME OCCURRED: 05/11/201512:00 DATE/TIME REPORTED BLM: 05/12/2015 <br />16:45 <br />LOCATION: SESW, SEC. 26, T.1 S, R. 98W 6TH Meridian <br />COUNTY: Rio Blanco STATE: Colorado <br />WELUFACILITY NUMBER: N/A OPERATOR: Natural Soda LLC <br />SURFACE OWNERSHIP: <Tedera Indian Fee State <br />LEASE NUMBER: C-0118327 UNIT NAME/CA NUMBER: N/A <br />TYPE OF EVENT: Blow out Fire Fatality Injury Property Damage <br />Oil Spill Saltwater Spill Toxic Fluid Spill Gas Venting <br />Oil/Saltwater Spill Oil/Toxic Fluid Spill <br />Other specify): Process Water Spill <br />CAUSE OF EVENT: A flange of process water pipeline was loosened at gate valve to facilitate <br />cleaning out a pipeline restriction. Carboy containment was placed under the flange to contain <br />expected light spray. However, a larger quantity of water released from the loosened flange and <br />overflowed the containment carboy. <br />VOLUMES OF MATERIAL/LIQUID: Discharged: 600 gallons of water <br />Recovered: approximately 500 aallons was recovered and <br />transported to plant evaporation pond. The water was contained <br />on currently disturbed ground. The affected disturbed area was 1000 <br />sq. ft. between pipeline configuration and in containment ditch adjacent <br />to existing plant road. No vegetation was affected. <br />TIME REQUIRED TO CONTROL EVENT (in hours): 0.3 hours <br />ACTION TAKEN TO CONTROL EVENT, DESCRIPTION OF DAMAGE, CLEAN UP <br />PROCEDURES AND DATES: Team was prepared to take action if operation did not proceed <br />as planned. Release from pipeline was stopped by re -tightening flange bolts to stop flow of <br />water. Released water was contained by building small dike by hand shovel within barrow ditch <br />adjacent to plant road. There was no release to nor damage of nearby sage and grass field. <br />Residual water filtered to ground within disturbed area, leaving minor residue of baking soda on <br />ground surface between pipeline configuration and within ditch containment. Baking soda <br />residue was scrapped off with shovel and skid steer, transferred to dumpster for routine disposal <br />in landfill. Spill clean-up operations were concluded by 17:00, 12 -May 2015, and inspected for <br />effectiveness.
