State the grass, forb, shrub and tree species to be planted and applicable quantities. <br /> Specify the quantity of each grass and forb species as pounds of pure live seed per <br /> acre. <br /> d. Specify the application method for grass and forb seeding. If the seed is to be <br /> broadcast, the application rate shall be twice the rate required for drill seeding. <br /> e. If mulch is needed, specify the kind to be sued, the crimping method, and rate of <br /> application. The Division recommends applying hay mulch at a rate of two tons per <br /> acre. <br /> f. Explain the establishment methods for each species of shrub and/or tree, and state the <br /> number of each to be established per acre. <br /> 8. The Applicant has stated the slopes on the proposed mine site do not exceed three percent. <br /> As noted under Item No. 6, it appears the existing slopes at the proposed mine site are steeper <br /> than three percent. As required by Rule 6.3.4(1)(b), specify the maximum gradient of <br /> reclaimed slopes (horizontal:vertical). <br /> 9. As required by Rule 6.3.4(1)(a), specify at what point in the mining plan and to what depth(s) <br /> overburden will be replaced in relation to ongoing extraction. The Applicant's response <br /> should reference the commitment to not have more than four excavations (2'W x 10'L x <br /> 10'D) and associated stockpile areas disturbed at any given time. <br /> 6.3.5 Exhibit E—Map <br /> 10. The Mining and Reclamation Plan Maps which were submitted for Exhibit E were not to the <br /> required scale. As required by Rule 6.2.1(2)(e), the acceptable range of map scales shall not <br /> be larger than 1 inch= 50 feet nor smaller than 1 inch= 660 feet. Please revise the Mining <br /> and Reclamation Plan Maps accordingly. <br /> 11. The Mining Plan Map must be revised to include the following elements: <br /> a) The name of the Applicant(s). <br /> b) The map must be signed by the person who prepared it. <br /> c) Give the date prepared. <br /> d) Label the permit boundary. <br /> e) Include a north arrow. <br /> f) The map must include a scale. Alternately, the Applicant can specify the length of each <br /> side of the permit boundary. <br /> g) Label the names of the owner(s) of record of the surface of the affected area and of the <br /> land within two hundred feet of the affected area. If all surrounding land is owned by the <br /> US Forest Service, then you may make a note of such on the map. <br /> h) Outline and label overburden stockpiles. <br /> i) Outline and label topsoil stockpiles. <br />
