Please identify the owner of record of the surface on the Mining Plan <br /> Map. <br /> 13. As required by Rule 6.3.5(2)(f), all existing disturbance within and/or adjacent to the permit <br /> boundary must be outlined and labeled on the Mining Plan Map. Re-disturbance of <br /> previously disturbed areas,by the proposed mining operation, must be included in the permit <br /> area and addressed in Exhibit D—Reclamation Plan. <br /> 14. The Mining Plan Map states the west half of the Rise Above claim (CMC 285712) <br /> corresponds to the permit boundary. The Rise Above claim is 20.66 acres in size, which <br /> makes half of the claim is 10.33 acres. The Applicant has specified the permitted area is 9.9 <br /> acres. Please remove the statement from the Mining Plan Map and ensure the permit <br /> boundary delineates an area of exactly 9.9 acres. <br /> 15. The Reclamation Plan Map must be revised to include the following elements: <br /> a) The name of the Applicant. <br /> b) The map must be signed by the person who prepared it. <br /> c) Give the date prepared. <br /> d) Label the permit boundary. <br /> e) Include a north arrow. <br /> f) The map must include a scale. Alternately, the Applicant can specify the length of each <br /> side of the permit boundary. <br /> g) State the average thickness of replaced overburden; you may make a note on the map that <br /> states how much overburden will be replaced. <br /> h) State the average thickness of replaced topsoil. <br /> i) Show the gradient of all reclaimed slopes (horizontal:vertical) sufficient to describe the <br /> post-mine topography. <br /> 6.3.6 Exhibit F—List of Other Permits and Licenses Required <br /> 16. Please indicate if you have contacted the Colorado Department of Public Health and <br /> Environment—Water Quality Control Division in regard to the need for a stormwater <br /> discharge permit. If you have not already done so, contact the Water Quality Control <br /> Division to determine if you are required to have a stormwater discharge permit. <br /> 6.3.7 Exhibit G—Source of Legal Right to Enter <br /> 17. The Applicant has provided a copy of the Location Certificate for the Rise Above claim. Please <br /> note the Division considers the approved Plan of Operations (P00)to be the Applicant's legal <br /> right to enter the property to conduct mining. Therefore, no mining may be conducted until the <br /> Applicant obtains an approved Reclamation Permit from the Division and an <br />
