December 4, 2014 C- 1981- 008/New Horizon Mine BFB <br />Pond 11 contained water and was discharging through the discharge flume. The flow indicator read .35 (units <br />unknown). <br />The debris at the inlet and outlet of culverts 111 and 109 has been removed but the pipes need to be cleaned. <br />Culverts 108 and 121 need to be cleaned. <br />Culverts 107, 120, 122, 123 and 124 have been cleaned. <br />A section of ditch C18 has been mowed to remove the cattails, but the ditch is still dense with vegetation (photo <br />6). The ditch is also filled with water that is not flowing due to the dense vegetation. Mowing alone will not be <br />adequate to re- establish the water flow that is needed to convey water to pond 007. The ditch configuration as <br />described in Attachment 2.05.3(3) -1 -32 needs to be re- established. <br />A culvert was in ditch C18 approximately halfway between the sump and ditch C4. The culvert is not shown on <br />Map 2.05.3(3) -1. <br />Ditch C4 is dense with cottonwood trees and willows (photo 7). The trees need to be removed to facilitate water <br />flow or the ditch can be reconstructed to its approved specifications (Attachment 2.05.3(3) -1 -14) 10 feet to the <br />east of its current position. <br />OTHER (SPECIFY): <br />A trench approximately 3' by 3' was dug between the BB Road and pond 11 (photo 8). The trench did not <br />interfere with the pond berm. The trench was dug to facilitate an irrigation drain line for the irrigation system on <br />the Morgan property. <br />I met with Rick Wolney, mine manager, to discuss the concerns raised by Elmer Burbridge. These concerns are <br />list in the November 2014 inspection report. Mr. Wolney is going to look into the irrigation issue to determine if <br />the irrigation piping is adequate to handle Mr. Burbridge's water shares and the condition of the water wells. <br />Western Fuels is going to re -seed the pond 8 embankment. <br />I will look into where pond 8 is in the bond release process to address the grazing issue and increasing the water <br />level in the pond. I will also look into the possibility of burying the black irrigation pipe into the embankment. <br />We will meet again during the next inspection to determine a course of action. <br />ROADS — Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and Maintenance4.03.1(5) and <br />(6)/4.03.2(5) and (6); Reclamation 4.03.1(7)/4.03.2(7): <br />The main haul road was well maintained.
