October 28, 2014 C- 1981 - 022 /Elk Creek Mine JRS <br />surrounding undisturbed areas. The pad is very steep, is in a native drainage and the soil is very sandy. The pad <br />was well vegetated. While there was some minor rilling on the pad, it did not appear to be a problem at this time. <br />There is a small seep below the reclaim but it does not appear to be causing any problems at this time. There were <br />no offsite impacts noted. <br />- GVB 11 -02 is a steep, reclaimed GVB pad. The well was sealed (there was soil in the casing to the surface) but <br />the casing has not been cut below the ground surface as required. The pad was well graded and stable and blends <br />well with the surrounding undisturbed areas. There was good vegetation on the reclamation. There was some <br />Mullen on the reclamation that will need to be sprayed next spring. There were no erosional problems or offsite <br />impacts noted on the reclaimed pad or surrounding undisturbed areas. <br />- GVB 10 -04 is a reclaimed GVB pad. The well was sealed, the casing was cut below the ground surface and <br />there was a borehole location marker in place. The pad was stable and there was well blended with the <br />surrounding undisturbed areas. There was good vegetation and no erosional problems were noted. There was <br />some Mullen on the site that will need to be sprayed next spring. There were no offsite impacts noted. <br />- GVB 09 -04 is a reclaimed GVB pad. The well was sealed with soil to the surface, the casing was at the ground <br />surface (and surrounded by rocks) and there was a borehole location marker in place. The pad was stable and <br />there was well blended with the surrounding undisturbed areas. There was good vegetation and no erosional <br />problems were noted. There was some Mullen on the site that will need to be sprayed next spring. There were no <br />offsite impacts noted. <br />- GVB 04 -02 is a reclaimed drill pad. The drill hole has been sealed and a borehole location marker was in place <br />as required. The slopes were very steep and blend well with the surrounding undisturbed areas. The pad is very <br />well vegetated and there were no signs of erosion on the pad. No offsite impacts noted at this site. <br />- GVB 03 -02 is a reclaimed drill pad. No offsite impacts noted at this site. <br />- The pad for dewatering Hole 1/2 was stable at the time of the inspection. There was a small slump on the upper <br />cut -slope of the pad (along the access road) but it does not appear that it has moved in some time. It will need to <br />be continually monitored by the operator. Hole DW -I was not grouted to the surface. Hole DW -2 was grouted to <br />the surface. There were straw wattles in the ditch and it appeared to be stable. The outslopes of the pad were <br />seeded this fall. The operator has placed straw wattles along the toe of the outslopes for sediment control. The <br />topsoil stockpile was stable and there was a sign in place. There was decent vegetation on the pile and it will <br />benefit from a second growing season. There were no offsite impacts noted at this pad. <br />- The pad for hole TGA was stable at the time of the inspection. This hole is a gas sampling hole related to the <br />mine fire. There were straw wattles in place in the ditch and along the outslopes of the pad. The topsoil stockpile <br />was stable and was recently seeded. There was a sign in place on the topsoil pile. There were no offsite impacts <br />noted at this pad. <br />- The pad for hole HGA was stable at the time of the inspection. This hole is a gas sampling hole related to the <br />mine fire. There were straw wattles in place in the ditch and along the outslopes of the pad. The topsoil stockpile <br />was stable and was recently seeded. There was a sign in place on the topsoil pile. There were no offsite impacts <br />noted at this pad. There was one problem on this pad related to the topsoil stockpile. The stockpile is below the <br />pad and, during pad construction, the operator appears to have cut into a native spring. The topsoil stockpile is <br />causing water to be impounded between the pad and the topsoil stockpile. This has been an ongoing issue, as there <br />are cattails growing in the impounded water.
