October 28, 2014 C- 1981 - 022 /Elk Creek Mine JRS <br />- GVB 17 -09 has been reclaimed. The reclaimed pad was very well vegetated and the surface was left in a very <br />rough condition. The hole was sealed and capped and there was a marker in place. There was some Mullen noted <br />on the pad. This will need to be sprayed in the spring. There were no erosional problems noted on the pad and no <br />offsite impacts were noted. <br />- GVB 17- 04/04B/04C is a reclaimed drill pad that contained three GVB's. All of the holes have been sealed and <br />were marked with borehole location markers. The reclaimed pad blends in well with the surrounding undisturbed <br />areas and there were no signs of any slumping, settling or cracking. The pad had lots of rocks but was well <br />vegetated and stable and no erosional problems were noted. There were no offsite impacts noted on this pad. <br />- GVB 16 -08 is a small pad along the light -use road. This pad and well have not been reclaimed. The well was <br />temporarily sealed but the annulus does not appear to be grouted to the surface. There were straw wattles in the <br />ditch and along the outslopes of the pad. The topsoil stockpile was very well vegetated and stable and there was a <br />sign in place. There were no erosional problems noted on the pad and no offsite impacts were noted. <br />- GVB 16- 04/16 -04B is shown on Drawing 2.05 -M1 Sheet 6 of 6 as a reclaimed drill pad. Both wells have been <br />sealed and are marked with borehole location markers. The pad also has a good amount of vegetation on it. <br />However, there are two remaining topsoil stockpiles, which make it appear that the pad was regraded but the <br />topsoil has not been spread on it. The pad blended in well with the surrounding undisturbed areas and there were <br />no erosional problems noted. There were no offsite impacts noted at this drill pad location. <br />- GVB 16- 05/0513/05C is a reclaimed drill pad that had three GVB's on it. All of the wells have been sealed and <br />are marked with borehole location markers. The pad blends in well with the surrounding undisturbed areas. The <br />surface was left in a roughened state and, although it has been hammered by cattle, it has good vegetation and it is <br />stable. There are no erosional problems noted on this pad. There is a topsoil stockpile that remains for the <br />reclamation of the road. The topsoil stockpile was well vegetated and stable and no erosional problems were <br />noted. There was a sign in place on the stockpile. There were no offsite impacts noted on this drill pad. <br />- GVB 16 -06 is a steep, reclaimed drill pad. The well has been sealed and there is a borehole location marker in <br />place. The pad blends well with the surrounding undisturbed areas. The pad is very well vegetated and there are <br />rocks and slash on the pad to help control erosion. There were no signs of erosion on the pad and no offsite <br />impacts noted. There was a short, reclaimed road spur to this pad. The spur also blended well with the <br />surrounding undisturbed. It was well vegetated and stable and no erosional problems or offsite impacts were noted <br />on the reclaimed road spur. <br />- GVB 15 -08 is a reclaimed drill pad. The well has been sealed and a borehole location marker is in place. The <br />pad blends well with the surrounding undisturbed areas. There is a small drainage channel at the west edge of the <br />reclaimed pad that remains stable. The pad is very well vegetated and there were no signs of erosion. There was a <br />lot of Gamble's oak growing at the upper edge of the pad. There were no offsite impacts noted. <br />- GVB 15 -07 is a reclaimed drill pad. The well has at least been partially sealed but not to within three feet of the <br />surface as required. Additionally, the well was not capped and the casing was not cut off below the ground surface <br />(there was a large rock sitting on the top of the well casing acting as a "cap "). The pad blends well with the <br />surrounding undisturbed areas. The pad is very steep, is in a native drainage and the soil is very sandy. The pad <br />was well vegetated. While there was some minor rilling on the pad, it did not appear to be a problem at this time. <br />There were no offsite impacts noted. <br />- GVB 15 -06 is a reclaimed drill pad. The well has at least been partially sealed, as the casing was filled with soil <br />to the surface. Additionally, the well was not capped and the casing was not cut off below the ground surface <br />(there was a large rock sitting on the top of the well casing acting as a "cap ").
