Any <br />stipulations associated with this permit and issued over the life of this operation which are not <br />discussed in this midterm review have been complied with or terminated. The following table <br />lists the stipulations placed on the permit since the last renewal, and the current status of those <br />stipulations. <br />Number <br />Description <br />Originating <br />Permit <br />Action <br />Status <br />29 <br />Prior to placement of cover on the eastern portion of Gob Pile #4, <br />TR -75 <br />CW <br />BRL must survey the eastern portion of the pile to verify that <br />slopes are not steeper than 2:1 and regrade any slopes as <br />necessary to ensure conformity with the approved slope design. <br />Documentation of the corrected slopes must be submitted to and <br />approved by the Division before cover can be placed on the <br />eastern portion of the disposal area. The area to be surveyed and <br />regraded is depicted on the February 8, 2013 review drawing <br />submitted with the Technical Revision No. 75 application. <br />30 <br />The 14,000 cy of cover fill material approved to be moved from <br />TR -83 <br />CW <br />the B Portal storage area must be direct hauled to, and used as <br />permanent cover material for, regraded portions of Gob Pile #4. <br />Page 9 of 22 <br />
