September 22, 2014 C- 1981 - 044 /Williams Fork Mines JLE <br />The sediment ponds were observed during the inspection. The majority of the ponds were dry with the exception <br />of a few damp puddles in a few of the ponds. The embankments of the ponds were vegetated and stable, however <br />a few rodent holes were observed that will need to be monitored and filled. Ponds 9A -P3 and 5A -P1 have shrubs <br />that have grown around the dewatering pipe gate that may impede the discharge of stored water and hinder the <br />functionality of the gate. Also, many of the emergency spillways have shrubs growing in them impeding their <br />functionality. The emergency spillway at Pond 9A -P2 was difficult to locate given the shrubs established in it. <br />Rule 4.05.9(2) requires these impoundments and spillways to function as designed and Rule 4.05.9(l 1) requires <br />maintenance of these structures. MCM must remove the brush hindering any dewatering device and obstructing <br />the spillways. <br />Water from the spoil spring was not discharging at the v -notch weir at discharge point 022 as depicted on Map 26. <br />ROADS — Rule 4.03, Construction 4.03.1(3)/4.03.2(3), Drainage 4.03.1(4)/4.03.2(4), Surfacing and <br />Maintenance4.03.1(5) and (6)/4.03.2(5) and (6), Reclamation 4.03.1(7)/4.03.2(7): <br />The roads were graded and maintained. <br />REVEGETATION — Rule 4.15 <br />Vegetative Cover; Timing: <br />Knapweed and Canada thistle patches were observed along diversion ditch 5 -D2. This area will need to be <br />monitored and treated in the future. <br />The Williams Fork Strip Pit was observed. A large patch of cheatgrass was observed on the north side of the <br />drainage and will need to be treated. Overall, the perennial grasses appear to dominate the strip pit area and <br />sagebrush has volunteered throughout the site. <br />SLIDES and DAMAGE - Rule 4.12: <br />The Portal No. 5 backfilled area was observed. This area appears stable and vegetated. <br />The area around the inlet and outlet of culvert 63 has been cleared and excavated. However, the culvert is about <br />3/4 full of sediment. Mr. Watterson indicated that MCM intends to replace this culvert. Rule 4.05.3(1) requires <br />that diversions and collections drains be maintained. When this culvert is replaced it must meet the specifications <br />outline in Exhibit 18 of the Williams Fork Mines permit application. <br />SUPPORT FACILITIES - Rule 4.04: <br />The two bridges crossing the Williams Fork River near its confluence with the Yampa River was observed and no <br />problems were noted. <br />The mine facilities were observed, no problems were noted. <br />Number of Partial Inspection this Fiscal Year: 0 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 3 of 6 <br />
