August 11, 2014 C- 1996 - 084 /Lorencito Canyon Mine LDS <br />expected given the recent work around the pond. The two rows of bales beside the access road were full of <br />sediment, to the extent that water had been running right over the bales and eroding the down -slope side. The <br />sediment should be cleared and the bales replaced if necessary. Beside culvert C 19 it appeared as though a <br />significant amount of water had cascaded off the road and had moved material around - as photo 4 shows, chunks <br />of vegetated soil had been moved around with the rocks. At the time of the inspection it did not appear as though <br />the integrity of the road had been compromised, but this area should be closely monitored for further damage, <br />particularly after heavy rain. <br />Pond 8 was holding water, but the access road was too soft to be easily passable by vehicle, so a close <br />inspection was not performed. <br />Pond 5 was holding a lot of water (all the weep -holes were submerged), but was discharging very slowly, <br />by drips. It appeared that there may be a problem with the primary spillway. There was some evidence that the <br />pond may have discharged over the emergency spillway. NECC should inspect the primary riser and spillway <br />as soon as conditions allow, and take any corrective action that may be necessary. <br />On the road above the shrub plot some grading had been carried out to prevent sheet flow of water across <br />the road, and some erosion on the slope between the road and the shrub plot had been repaired. It may be that a <br />culvert is needed for a long term solution (approximately where the car is parked in photo 13), particularly if silt <br />continues to build up beside the road as it did before. This should be monitored over the next few months. <br />Above pond 6, the ditch traversing the top of the steep slope had been cut and seeded. Some straw wattles <br />had been laid across the slope just below the ditch, but the lower part of the slope had not yet been addressed. <br />The bales and the check dams beside the road were in the process of being cleaned out. The low spot of the pond <br />embankment, beside the emergency spillway, that has been referred to in multiple inspection reports, had been <br />graded and seeded and looked excellent; the grading will ensure that all run -off ends up in the pond and the seed <br />was already germinating. The pond was holding water but not discharging. <br />Pond 9A was holding water, but not discharging. The downdrain on the west side of fill 9 had been <br />damaged where intense rain had moved very large rocks around leaving several sections with large drop -offs, <br />maybe 8 -10' high. The entire length of the down drain needs attention as soon as possible to ensure that the <br />slopes and terraces of the fill are not damaged. <br />On the downdrain on the east side of fill 7, similar damage had occurred, although not quite as bad. At the <br />very top of the downdrain, just below the outlet of culvert C4, the ditch had been repaired and seeded. Pond 7 <br />looked good. <br />GENERAL MINE PLAN COMPLIANCE: <br />TR18 - The proposed decision due date is August 29, 2014. The Division is awaiting a response to the <br />adequacy letter of May 29, 2013. <br />ROADS — Rule 4.03 <br />Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and Maintenance 4.03.1(5) and <br />(6)/4.03.2(5) and (6); Reclamation 4.03.1(7)/4.03.2(7): <br />Roads were generally in decent condition, however the roads leading to the pond cleaning area from ponds <br />9 (913) and 6 were quite rutted following the repeated passage of the haul truck.
