August 18, 2014 C- 1981 - 010 /Trapper Mine JLE <br />were observed. Small center channels have begun to form in these drainages, but overall they appear stable and <br />functional. <br />EXPLOSIVES — Rule 4.08 <br />Distance Prohibitions 4.08.4; Warnings 4.08.4; Control of Adverse Effects 4.08.4: <br />A blast was observed in the K- Strike pit area. All of the warning signals were completed and access to the area <br />was blocked off. <br />HYDROLOGIC BALANCE - Rule 4.05; Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6, Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water <br />Monitoring 4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The Division observed the sediment ponds and the NPDES discharge ponds /points during the inspection. Overall, <br />the pond embankments appeared stable and vegetated. The primary and emergency spillways were clear and <br />functional. Trapper has been conducting pond cleaning /maintenance operations at several of the ponds discussed <br />below. <br />No Name No. 2 pond water was pumped down so the pond can be cleaned. The No Name No. 4 spillway culvert <br />riser is being repaired, the vertical portion of the pipe is removed and water is discharging from the horizontal <br />portion of the spillway. The water elevation of No Name No. 4 pond is low as a result of this operation and the <br />pond will be cleaned and excess sediment removed. <br />Oak pond No. 1 was being cleaned at the time of the inspection. <br />West Flume No. 1, Middle Flume No. 1 and East Flume pond was recently cleaned. <br />Water was discharging from the NPDES discharge points for No Name Gulch and East Pyeatt Gulch. However, <br />the East Pyeatt No. 3 pond did not appear to be discharging, the water was coming from the nearby spring. <br />The following culverts were observed and found to be clear and functional: Culverts A -2, A -4, A -5, GRS -1, GRS- <br />2, A -14, AE -10, AE -4, SA -7. <br />Culvert A -9 was half full of sediment at the outlet end of the culvert and will need to be cleaned. <br />Culvert AE -7 was half full of sediment at the outlet end of the culvert and will need to be cleaned. <br />Culvert A -13 was buried at the inlet of the culvert. Trapper, excavated the buried portion of the culvert and <br />cleaned out the culvert at the time of the inspection. <br />Trapper has reclaimed the contour ditches across portions of the F -Pit reclamation parcels where vegetation has <br />Number of Partial Inspection this Fiscal Year: 1 <br />Number of Complete Inspections this Fiscal Year: 1 <br />Page 3 of 24 <br />
