The Geoweb is a honeycomb of plastic material that is staked to the channel bottom and then filled with <br />riprap to secure the rock material from wash out during any substantial precipitation events. The <br />Geo7eb appeared to be installed correctly. This drainage ditch is roughly 4 -5' wide. <br />Conversely, the As -built indicates that 3" and 6" riprap has been used for armoring in the NHN -1 <br />drainage. This does not match what our field inspection revealed. Furthermore, appendix 2.05.3(4) -1 <br />"Ar adis Report on Sediment Control" states that all of the ditches will be constructed resulting in a <br />bott m width of a minimum of 12'. As -built reports widths that range from 3' -17'. This doesn't match <br />what is in the field. <br />Because of the discrepancies identified, DRMS has issued a Notice of Violation (NOV) to the operator <br />and included the required abatement steps necessary. <br />NHI -1 Sediment Pond <br />The pond was currently holding water but not discharging at the time of inspection. There are several <br />areas (in slope) where rills /gullies have formed due to the lack of vegetation establishment and erosion <br />control BMP's. The operator informed us that this pond has been in place for 2.5 years. <br />The emergency spillway and the principal spillway have been constructed as designed and meet all <br />requirements. The principal spillway had a trash rack in place and free of any debris at the time of <br />inspection. Head cutting into the embankment around the principal spillway was evident and will <br />require maintenance as well as the east side inflow area. <br />All ulverts were free of sediment and debris with the exception the culvert on the south side of mine <br />enuring into the equipment staging area (near NHN -1 pond). This culvert needs to be cleaned out to <br />remove excessive tumbleweed accumulation. This area of the NHN -1 drainage had an impermeable <br />"rubber" liner that has been installed in lieu of riprap to prevent any erosion or down cutting. Although <br />this lmaterial seemed to be adequate, this is not in the approved permit. <br />Topsoil (Rule 4.06): <br />Topsoil did not have any vegetation but did have perimeter drainage BMP in place to mitigate for water <br />erosion. The operator indicated they don't have plans to seed until the fall. However, Section <br />2.05.4(2)(d) of the approved permit provides specifics as to planting vegetation on stockpiles that have <br />been created during the summer months. <br />Signs and Markers (Rule 4.02): <br />The mine identification sign located at the mine site entrance on the west side contained all necessary <br />information in accordance with 4.02.2 but will need to be replaced on an appropriate post so that it can <br />easily be found and identified. Blasting signs and permit boundary markers were observed to be easily <br />visible along the fence that also acts as permit boundary marker. One topsoil sign was found identifying <br />the stockpile. 1x2 stakes have been used to identify the perimeter of disturbance at the mine but <br />visibility is minimal. <br />The following items were verified on file at the New Horizon North Mine office during the records check: <br />• DRMS Permit <br />• Permit Application Package with revisions <br />
