June 10, 2014 C- 1982 - 056 /Foidel Creek Mine JDM <br />- The erosion control measures at the discharge of Pond D will require maintenance. The silt fence and straw <br />waddles are no longer adequate. The silt fence has been buried and displaced and the waddles are displaced. <br />Maintenance of the BMPs will be required or reinstallation of the BMPs is required. If Twentymile Coal believes <br />that the BMPs are no longer required please provide evidence to the Division of adequate protection of the area <br />from disturbance. <br />Ditches and culverts throughout the mine site were functioning as designed. No problems were noted with the <br />exception of the following: <br />- The outlet of third culvert from the northern most end of the 18RT access road was blocked. Evidence of <br />clearing the culvert was observed; however, the culvert remains partially blocked and damaged at the outlet. <br />- The first culvert along the barricade chamber access road from the Fish Creek Borehole access road has been <br />removed and a small ditch allows the water to cross the road. This culvert will require re- installation or a <br />demonstration to the Division will be required to discuss the removal of the culvert at this location. <br />- The second culvert along the barricade chamber access road on the south side is damaged at the inlet and will <br />require maintenance. <br />- The third culvert on the barricade chamber access road on the north side is down - cutting at the outlet. This will <br />need to be repaired or reinforced with a material suitable to prevent erosion at the outlet. <br />- The fourth culvert on the barricade chamber access road is damaged at the inlet and —1/2 full of material at the <br />outlet. This culvert will require maintenance. <br />- Three — 8" pipes are identified under the 6MN access road to aid in the drainage of water in the area. These <br />pipes are not identified on Map 24 and no designs for these pipes exist. <br />- Heavy down- cutting remains at the 6MN Pad diversion ditch to the 6MN sediment pond, with gullies on the <br />slopes of the 6MN pad to the collection ditch. This area will need to be address to repair erosion features. <br />- The culvert under the 6MN access road at the NW Vent shaft site was —1/2 full of material at the inlet. This will <br />require cleaning. <br />- The first culvert on the 18RT Vent Shaft access road will require cleaning at the inlet to remove material. <br />- The culvert at the CR27 rock dust tank pad will require clearing at the outlet to remove material. <br />- The explosives area bunker access road culvert has been crushed and buried at the outlet. This culvert will <br />require repair. <br />- A possible sediment sump has been cut in on the north side of the train load -out access road near Pond C <br />unclear if this is a possible sediment sump as the work does not allow flow to the possible sump area.
