June 2, 2014 C- 1981 - 010 /Trapper Mine JLE <br />forms were missing the distance measures. Trapper will need to update these forms with the correct information. <br />The Operator sought to renew their explosives license /permit through the Bureau of Alcohol, Tobacco, Firearms <br />and Explosives. Trapper's records contain a letter from this agency indicating they received the Operators renewal <br />application and that they have not completed processing the application. The letter goes on to say that Trapper can <br />continue to operate under their current permit until the Bureau has processed their renewal application. <br />BACKFILL and GRADING — Rule 4.14; Contemporaneous Reclamation 4.14.1; Approximate Original Contour <br />4.14.2; Highwall Elimination 4.14.1(2)(f); Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials <br />4.14.3; Stabilization of Rills and Gullies 4.14.6: <br />The Operator is concurrently backfilling behind the mining operations. <br />Fly ash is continuing to be backfilled in the Ash Pit. No problems were observed. Parcels A -B -12 and A -A -13 <br />were observed. These parcels appeared stable, some minor rills have formed but large rills and gullies were not <br />observed. <br />EXCESS SPOIL and DEVELOPMENT WASTE — Rule 4.09, Placement; Drainage Control; Surface Stabilization: <br />Horse Gulch Fill was observed. The face of the fill was stable and vegetated. The side drains of the fill appeared <br />stable and functional. <br />EXPLOSIVES — Rule 4.08; Distance Prohibitions 4.08.4; Warnings 4.08.4; Control of Adverse Effects 4.08.4: <br />Please see the Availability of Records section of the permit for a discussion of corrections that need to be made to <br />the blast summary forms. <br />While inspecting the development drill pad 2013 -AE2 the Operator was conducting a blast in the main pit area. <br />The appropriate radio warning communications were announced and the warning sirens could be heard from this <br />location. After the blast, Mr. Roberts took me to the approximate location of the blast. I marked the location with <br />my GPS unit as well as the location of the development drill pad. The drill pad was about 3850 away from the <br />blasting area. Rule 4.08.4(3) requires that the warning and all -clear signals be audible within range of one half <br />mile from the point of the blast. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Each of the ponds at the end of the drainage system was inspected. Graham Roberts conducted Trapper's monthly <br />sampling of each of the NPDES discharge points and springs at the site. Prior to the inspection, I witnessed Mr. <br />Roberts calibrate his water meter that measures pH, temperature and conductivity.
