I, Jared Ebert of the Colorado Division of Reclamation, Mining and Safety conducted the inspection. Mr. <br />Graham Roberts with Trapper Mine accompanied me during the inspection. The weather was warm and mostly <br />sunny during the inspection. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />The Division conducted a records check at the mine office on Monday June 2, 2014. See the last page of the <br />report for a summary of the Division's findings. <br />The Operator conducts airblast monitoring in accordance with Rule 4.08.4(6). Prior to the approval of TR112, <br />Trapper utilized two Instantel Inc. Minimate® airblast monitoring units. The results of monitoring are supplied to <br />Trapper by Matheson Mining Consultants, Inc. In general, it appears Matheson Mining Consultants supplies the <br />Operator with monitoring results on a quarterly basis. Reports from monitoring from January 2011 until <br />September 2013 were available for inspection. However, the Division could not locate reports from April 1, 2012 <br />through June 30th, 2012 and January 1, 2013 through June 30, 2013. <br />The last report received by Trapper Mining was dated October 15, 2013. The report indicated that no blasting <br />related vibration events occurred during July 1, 2013 to September 30, 2013. The last reportable blasting related <br />vibration occurred on December 14, 2012. The airblast was reported to be 101.9 dB(L), which is below the <br />maximum level allowed by Rule 4.08.4(6). <br />Trapper has three years of blast summary records available at the mine office. Rule 4.08.5(4) requires that the <br />identification, direction and distance, in feet from the nearest blast hole to the nearest dwelling, school, church, or <br />community or institutional building not located in the permit area or owned or leased by the person who conducts <br />the surface coal mining operations be reported on the form. The majority of the forms include the distance to the <br />nearest building but the direction and identification of the structure was not reported on the forms.
