6.4 SPECIFIC EXHIBIT REQUIREMENTS — REGULAR 112 OPERATIONS <br />The following items must be addressed by the applicant in order to satisfy the requirements of C.R.S. 34- <br />32.5 -101 et seq. and the Mineral Rules and Regulations of the Mined Land Reclamation Board <br />6.4.3 Exhibit C - Pre - Mining and Mining Plan Maps of Affected Land <br />4. Please provide an updated map showing the actual location of the Logan County Kokes Pit (M -1978- <br />249) boundaries as compared to the proposed expansion boundaries of the MMM Partnership Gravel <br />Pit. Enclosed is a copy of the survey plat for the approved Kokes Pit boundaries. This was submitted <br />on 10/25/2001 and can be found on the Division's image document database at the following website: <br />http://dnnsweblink.state.co.us/drrnsweblink/search.aspx?dbid=O. <br />5. As required by Rule 6.2.1, all maps must be prepared and signed by a registered land surveyor, <br />professional engineer, or other qualified person. Please sign the Exhibit C maps and all other maps <br />submitted for the conversion application. <br />A map showing the relationship between the Logan County Kokes Pit (M- 1978 -249) and MMM <br />Partnership expanded pit (M- 2006 -042) is attached. The Logan County Kokes Pit was expanded <br />from the original survey map you have provided. All maps previously submitted are attached with <br />signatures. <br />6.4.4 Exhibit D - Mining Plan <br />6. Please provide a signed and notarized letter from the landowner stating the access road into the site <br />was pre- existing and shall remain after final reclamation of the mine complete. Please note that this <br />road is allowed to be maintained, but if the road is widened to accommodate two -way traffic an <br />amendment to the permit must be submitted to the Division to account for the increase in disturbance <br />area. <br />Mark Kokes, president of MMM Partnership, has provided a signed and notarized letter to the <br />Division regarding intent to maintain the existing access road. A copy is also attached with this <br />response. <br />7. Please state the depth and thickness of the deposit to be mined as required by Rule 6.4.4(f)(i). <br />Based upon experience in mining portions of the original 9.9 acre permit, deposits range from five <br />to fifteen feet in thickness. Depth to the material varies, but is often directly below the topsoil. <br />8. Please state the nature of the stratum immediately beneath the material to be mined as required by <br />Rule 6.4.4(f)(ii). <br />Based upon experience in mining portions of the original 9.9 -acre permit, the stratum immediately <br />beneath the material to be mined will be either clay or sand. <br />
