March 2014 Stonewall Springs — QA/QC Plan <br />7.0 FIELD DOCUMENTATION <br />7.1 DAILY FIELD REPORTS <br />The CQA Field Representative shall document field observations and the general results of field tests in <br />the daily field reports the following: <br />• Name <br />• Company name <br />• Date <br />• Start time and ending time <br />• Weather <br />• Job reference number <br />• Contractor or subcontractor onsite <br />• Major equipment onsite and /or used (or list) <br />• Page number <br />• Conversations related to project <br />• Activities performed that consumed the day <br />• Activities performed started or restarted that day <br />• Activities performed started or completed critical to the project <br />• Photographs of activities per section 7.3- Photographs <br />• Reference attached test results, sketches, photos, etc <br />• Location of activities including facility name, control line and station, offset distances, elevation, <br />and materials used and tested. <br />• Status of as -built drawings being maintained by Contractor <br />• Problems encountered and resolutions implemented <br />• Identification of potential claim issues and Change Orders <br />• Attach field test data sheets <br />7.2 FIELD TEST DATA SHEETS <br />All field testing will be recorded by the CQA Field Technicians and /or the CQA Field Representative on <br />Inspection Data Sheets. These sheets will be used to formulate the daily summary reports. All field <br />testing will follow ASTM or other specified test procedures and methods of data recording. Observations <br />in the field may be in the form of notes, charts, drawings or sketches, photographs, or any combination of <br />the above. The Inspection Data Sheets will contain the following information: <br />• Date, name of project and location <br />• Description and title of the construction and related inspection activity <br />• Time the activity was performed <br />• Location of the inspection activity <br />• Photographs of construction activity <br />• Weather and site conditions <br />• Standard test method used or type of inspection activity <br />• Test equipment used <br />• Record of observation and test data, with all calculations completed and checked <br />• Construction methods and equipment origin material and general description of conditions <br />• Comparison of test results and observations to the specification requirements and indicate Pass <br />or Fail <br />• Names and titles of all persons involved in the inspection activity <br />• Record of materials and workmanship that do not meet specified design and corrective action <br />measures and results taken <br />12 <br />
