Subsidence Monitoring Data <br />The operator of an underground mine should have copies of recent data to verify compliance <br />with the approved monitoring plan. <br />Rill and Gully Survey <br />The operator should have a copy of. the most recent report submitted to CDMG. <br />Vegetation Monitoring Data <br />Copies of data from required monitoring should be on file. <br />Specific Variance Approvals <br />If the operator has been granted a variance, a copy of that approval should be on file. <br />Annual Reclamation Reports <br />A copy of the latest report (submitted to CDMG) should be on file. <br />HYDROLOGIC RECORDS <br />NPDES Permit <br />NPDES permit should be current (check expiration date). If expired, renewal application letter <br />to CDOH should be available during CDOH review. NPDES permits contain sampling <br />requirements and effluent limits for discharges. The mine's hydrology monitoring requirements <br />(locations and frequency) and effluent limits (including storm exemptions) should, at a minimum, <br />include the NPDES sampling plan. <br />NPDES Records <br />Discharge Monitoring Reports (DMRs) should be available for the past year or so, up through <br />the most recent quarter (i.e., third quarter reports should be available during the fourth quarter <br />inspection). The reports should either list values for parameters listed in the NPDES permit or <br />say "no discharge." There should be a DMR for each permitted discharge point on the mine. <br />If there is an effluent limit excursion, there should be an explanation of the circumstances by <br />the operator in which they justify to CDOH that it was an isolated event and not cause for <br />enforcement action. Excursions should be recorded in CDMG inspection reports. <br />Storm Water Management Plan <br />The operator is required by CDOH to have a plan that addresses routing of stormwater runoff <br />from areas not draining to a sediment pond. A copy of this should be on file. <br />-2- <br />
