MINE RECORDS INSPECTION <br />PERMIT RECORDS <br />DMG Permit <br />Current permit should be on file. <br />Permit Application with Revisions <br />The approved permit application package including all permit revisions should be available . <br />Findings Document <br />A copy of the latest CDMG findings document should be on file. <br />Insurance Certificate <br />Proof of insurance must be available at the mine, with coverage limits as indicated in Rule <br />2.03.9. The insurance certificate must include. a designation of the insured and at which <br />operation, what activities are covered, that CDMG is the certificate holder, and a change or <br />cancellation notice clause. <br />Bond Document <br />A copy of the reclamation performance bond should be available at the mine. <br />Phased Bond Release Document and Findings <br />The operator should maintain complete copies of CDMG bond release documents. <br />Air Emission Permits <br />A copy of the current and /or final permit issued by CDOH should be on site. <br />County Special Use Permits <br />A copy of the current special use permit issued by the respective county must be on site. <br />Underground Mining Landowner Notification <br />The operator of an underground mine is required to have copies of and certified mail receipts <br />for landowner notification. Notification is required at lease 6 months prior to undermining. <br />Subsidence Monitoring Reports <br />The operator of an underground mine should have a copy of the most recent report submitted <br />to CDMG. <br />-1- <br />
