Mr. Greg Lewicki has been added to the list of persons who have prepared sections of
the original or subsequent permit documents.
7. Mr. Parker Newby, PE and President of Goff Engineering and Surveying has been
identified in the APD as the current Resident Agent for the Carbon Junction Mine for the
purposes of Rule 2.03.4(2)(b).
8. Section 2.03.5 text in the APD has been revised to reflect CV- 2013 -005. It is noted for
the record, that the APD text was accurate at the time it was submitted to CDRMS, and
that the subject NOV was issued during the review period of RN04.
2.05.3- Operation Plan- Permit Area
1. The text in the current Abridged Permit Document has been revised to reflect the text
references provided (Enclosure 1) by CDRMS. I would like to discuss the identified
exhibits and their topics during our March 25 meeting to ensure consistency between the
Division APD, the public copy of the APD at Goff Engineering, and my copy of the
APD.
2. A text reference to TR17 and Appendix 5 -12, and the plans and modification of the
Carbon Junction Canyon channel have been added to this section.
3. A text reference to TR17 and Appendix 5 -12, and the plans and modification of the
Carbon Junction Canyon channel have been added to this section.
2.05.4 — Reclamation Plan
1. In the original permit document copy I have on file, drill log information is contained
in Appendices 4 -2b and 4 -3. I did not find drill logs for wells 94 -1 and 95 -1 in the
original permit document, as you noted. In other information obtained from the Colorado
Division of Water Resources, I found drilling descriptions for these two holes. During
our March 25 meeting, let me show you the information I have from DWR, and
determine if this information addresses your comment.
Information with regard to the sealing of wells B17, 10B (AKA B10) and 94 -1 was
provided to DWR and the Division previously. DWR considers the abandonment
documentation acceptable and these files are closed.
2. Per an agreement with David Berry and Sandra Brown of CDRMS, a single revision
addressing the remaining reclamation issues at the Carbon Junction Mine will be
prepared and submitted in mid -2014. This revision will address the retention of the cited
road, revision of the revegetation success criteria, revisions related to the work at the
Carbon Junction Canyon channel and all other remaining final reclamation issues at the
mine.
