Cripple Creek & Victor Gold Mining Company <br />PSES Pipe Rack Liner Extension <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />February 4, 2014 <br />performed geosynthetics CQA activities in accordance with the Geosynthetics CQA <br />Plan (Section 1400.2 of the Technical Specifications located in Appendix E). <br />Geosynthetics CQA activities included: <br />• Observation and documentation of geomembrane deployment, trial seams, <br />seaming, destructive testing, and non - destructive testing (pressure tests and <br />vacuum box tests) <br />• Observation of GCL placement to ensure adequate panel overlap <br />• Third -party conformance testing of geomembrane <br />• Measured geomembrane panel thickness and confirming adequate panel overlap <br />• Visually inspected geomembrane sheet quality, sheet grinding, seam alignment, <br />and continuity <br />• Retained archive samples of geomembrane destructive tests <br />1.3 Project Coordination <br />CQC and CQA activities were directed by the AMEC Project Resident. The Project <br />Resident (CQA Supervisor) for the construction of the PSES Pipe Rack Liner <br />Extension from September 2013 through November 2013 was Mr. Thorne M. Clark. <br />The Certifying Engineer and Project Manager, Mr. Michael E. Nelson, PE, made <br />periodic site visits to resolve technical issues and to review AMEC activities. <br />Appendix C presents a summary of the AMEC staff on site between September 2013 <br />and November 2013. <br />Representatives of CC &V, Ames, ECA, and AMEC conducted pre- construction, daily, <br />and weekly coordination meetings throughout the construction period. <br />1.3.1 Daily Construction Summary Reports <br />AMEC prepared daily construction summary reports (Appendix D) that detail the <br />activities performed as well as any quality control activities performed on -site for each <br />day of the construction process. Photographic documentation (Appendix B) was also <br />kept throughout the project to document key elements of construction and the CQA <br />activities that occurred. <br />1.4 Project Specifications <br />All work was performed in accordance with the design plans and specifications <br />presented in Amendment No. 9 to Mining Permit No. M- 1980 -244 and subsequent <br />revisions, as approved the Colorado Division of Reclamation, Mining and Safety <br />(DBMS).
