Cripple Creek & Victor Gold Mining Company <br />PSES Pipe Rack Liner Extension <br />Final Report <br />Duality Assurance Monitoring & Test Results <br />February 4, 2014 <br />ameO <br />1.2 Construction Quality Assurance /Construction Quality Control <br />AMEC provided CQA and CQC monitoring and testing services, and prepared this <br />report describing the monitoring and test results for the PSES Pipe Rack Liner <br />Extension. As -built drawings and supporting CQA monitoring and testing <br />documentation are presented in appendices attached to this report. <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQC testing and CQA testing and monitoring of earthworks for the <br />PSES Pipe Rack Liner Extension. CQA testing and monitoring of earthworks was <br />provided in accordance with the AMEC Earthworks CQA Plan (Section 1400.1 of the <br />Specifications; located in Appendix E). The earthworks material specifications for this <br />project are summarized in Table 1. CQC earthworks testing was provided to support <br />the contractor during the manufacture of materials and prior to material placement. <br />The CQC test results are not included in this report as these tests were intended to <br />assist the contractor with materials preparation rather than record testing. All CQA <br />testing results are included. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks <br />construction activities as follows: <br />• AMEC visually monitored random fill subgrade preparation to confirm the subgrade <br />surface was unyielding, free of organic matter and other unsuitable materials. <br />• AMEC visually monitored the placement of Drain Cover Fill (DCF) above the <br />primary geomembrane in the PSES Pipe Rack Liner Extension, including <br />verification of lift thickness and grade control; identification and removal of <br />deleterious materials; and verification that care was exercised during fill placement <br />to avoid geomembrane damage. AMEC performed CQA laboratory testing of grain <br />size, Atterberg limits, and moisture contents. <br />• AMEC visually monitored the compaction of Drain Cover Fill (DCF) in accordance <br />with the specification amendment letter which can be found in Appendix E.2. <br />AMEC observed all compaction of DCF material to ensure that care was taken to <br />avoid geomembrane damage during compaction. <br />• AMEC monitored the ambient air temperatures and weather in accordance with <br />project specifications. The ambient air and daily weather observations during <br />construction activities are summarized in Table 2. <br />1.2.2 CQA/CQC Monitoring of Geosynthetics Installation <br />AMEC provided CQA monitoring of geosynthetics used for the PSES Pipe Rack Liner <br />Extension. CQA services consisted of performing geosynthetics inventory and <br />checking quality control certifications prior to geosynthetics deployment. AMEC <br />2 <br />
