Cripple Creek & Victor Gold Mining Company <br />Mill Site Earthworks Project <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 31, 2014 <br />TMA <br />supporting CQA monitoring and testing documentation are presented in appendices <br />attached to this report. <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQC testing and CQA testing and monitoring of earthworks for the Mill <br />Site Earthworks Project. CQA testing and monitoring of earthworks was provided for <br />in -place materials in accordance with the AMEC Earthworks CQA Plan (Section <br />1400.1 of the Project Specifications; see Appendix E). The earthworks material <br />specifications for this project are summarized in Table 1. CQC earthworks testing was <br />provided to support the contractor during the manufacture of materials and prior to <br />material placement. The CQC test results are not included in this report as these tests <br />were intended to assist the contractor in materials preparation rather than record <br />testing. All CQA testing results are included. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks <br />construction activities as follows: <br />• AMEC monitored topsoil stripping, site grading, including slope re- contouring and <br />fill placement, lift thickness, moisture conditioning, temperatures, compaction <br />efforts, in -place nuclear density testing, and use of suitable material for <br />Overburden Fill (OVF). AMEC performed CQA laboratory testing of grain size and <br />Atterberg limits. <br />• AMEC visually monitored the construction of the Secondary Underdrain, including <br />verification that Ames excavated trenches to the minimum required dimensions <br />and grades, and placed materials as specified. AMEC performed CQA laboratory <br />testing of grain size, Atterberg limits, and moisture content of the Underdrain Filter <br />Material (UFM) and Select Structural Fill (SSF). <br />• AMEC visually monitored and documented the remediation of underground <br />workings within the Mill Site Earthworks Project footprint, including verification that <br />Ames excavated underground workings to the minimum specified dimensions and <br />installed Coarse Shaft Backfill (CSB), cemented rockfill, concrete, geogrid, SSF, <br />and Structural Fill (SF) as specified on the Issued for Construction (IFC) drawings. <br />• AMEC visually monitored subgrade preparation to confirm the subgrade surface <br />was unyielding; free of organic matter and other unsuitable materials; and the <br />subgrade surface was compacted prior to Soil Liner Fill (SLF) placement. <br />• AMEC visually monitored the placement and compaction of SLF including depth <br />verification performed by Ames, particle size, moisture conditioning, and <br />compaction of the fill. In place nuclear moisture density testing was conducted as <br />well as CQA laboratory testing of grain size, moisture content, Atterberg limits, <br />Proctor, and permeability.
