Cripple Creek & Victor Gold Mining Company <br />Mill Site Earthworks Project <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 31, 2014 <br />ameO <br />moisture conditioning for geomembrane deployment, including verification that the <br />surface was proof rolled, compacted, and hand worked to be free of desiccation <br />cracks, abrupt grade changes, and sharp stones or objects. AMEC also observed <br />the excavation, backfill, and compaction of anchor trenches. <br />• AMEC visually monitored the placement of Drain Cover Fill (DCF) above the <br />geomembrane in the Mill Site Earthworks Project, including verification of lift <br />thickness and grade control; identification and removal of deleterious materials; <br />and verification that care was exercised during fill placement to avoid <br />geomembrane damage. AMEC performed CQA laboratory testing of grain size <br />and Atterberg limits. <br />• AMEC monitored the ambient air temperatures and temperatures of fills, including <br />SLF, in accordance with Project Specifications. The ambient air and fill <br />temperatures monitored during fill activities are summarized in Tables 2 and 3, <br />respectively. <br />1.2.2 CQA/CQC Monitoring of Geosynthetics Installation <br />AMEC provided CQA monitoring of geosynthetics used for the Mill Site Earthworks <br />Project. CQA services consisted of performing geosynthetics inventory and checking <br />quality control certifications prior to geosynthetics deployment. CQA activities were <br />performed in accordance with the Geosynthetics CQA Plan (Section 1400.2 of the <br />Project Specifications; located in Appendix E). Geosynthetics CQA activities included: <br />• Observation and documentation of geomembrane deployment, trial seams, <br />seaming, destructive testing, and non - destructive testing (pressure tests and <br />vacuum tests) <br />• Performed conformance testing of geomembrane <br />• Measured geomembrane panel thickness and confirming adequate panel overlap <br />• Visually inspected geomembrane sheet quality, sheet grinding, seam alignment, <br />and continuity <br />• Retained archive samples of geomembrane destructive tests <br />1.3 Project Coordination <br />CQC and CQA activities were directed by the AMEC Project Resident. The Project <br />Resident for the construction of the Mill Site Earthworks Project between October 2011 <br />and November 2012 was Mr. Joseph D. Hickey and for the period between November <br />2012 and December 2012, was Mr. Thorne Clark. The Project Manager, Mr. Jay <br />Janney- Moore, PE, or his representative, Mr. Ronald Arlian (Project Sponsor) made <br />3 <br />
