Cripple Creek & Victor Gold Mining Company <br />Mill Site Earthworks Project <br />Final Report ameO <br />Quality Assurance Monitoring &Test Results <br />January 31, 2014 <br />1.0 INTRODUCTION <br />The Cripple Creek & Victor Gold Mining Company (CC &V) operates the Cresson <br />Project; located in Teller County, Colorado, near the city of Victor. CC &V is owned <br />and managed by AngloGold Ashanti (Colorado) Corp. (AngloGold Ashanti). The <br />Cresson Project is a mining and ore processing facility which is comprised of surface <br />mines, crushers, a lined Valley Leach Facility (VLF), gold recovery plant, and <br />associated infrastructure, all of which were designed and constructed to meet or <br />exceed the requirements established by C.R.S. §34 32 101 et seq. and regulations <br />promulgated there under by the Mined Land Reclamation Board (MLRB). <br />The work associated with the Cresson Project is being performed under specific <br />criteria established in Amendment No. 9 of Permit Number M- 1980 -244 (Permit), as <br />approved by the MLRB. This report documents the Construction Quality Assurance <br />(CQA) and Construction Quality Control (CQC) services provided by AMEC <br />Environment & Infrastructure (AMEC) from October 2011 through December 2012 for <br />the Mill Site Earthworks Project, which is defined in Section 1.6. The general site plan <br />is presented on Figure 1, and the Construction and Geomembrane Installation Limits a <br />presented on Figure 2. <br />1.1 Project Description <br />The initial Cresson Project was designed in 1993 and constructed in phases since <br />1994 under Amendment Nos. 6, 7, 8, and 9 to Mining Permit M- 1980 -244. This <br />certification report addresses the addition of approximately 1,517,215 square feet plan <br />area of a portion of the earthen and synthetically lined area north of Phase IV. This <br />area is referred to as the Mill Site Earthworks Project, and was designed to meet the <br />criteria presented and approved in Amendment No. 9. <br />Ames Construction Inc. (Ames) was the earthworks contractor for the Mill Site <br />Earthworks Project. They performed construction activities for the Project from <br />October 2011 through December 2012. Poly -Flex, Inc. and GSE Environmental <br />manufactured the geomembrane used to construct the Mill Site Earthworks. Lone Star <br />Lining Company (Lone Star) completed the installation of geomembrane liner from <br />October 2012 through November 2012. Messrs. Ron Roberts, Jim Smith, and Scott <br />Redabaugh represented CC &V as the Construction Manager and Construction <br />Superintendents, respectively. All quantities and testing frequencies presented in this <br />report are current as of December 2012, which represents completion of the Mill Site <br />Earthworks Project, as detailed in this report. <br />1.2 Construction Quality Assurance /Construction Quality Control <br />AMEC provided CQA and CQC monitoring and testing services, and prepared this <br />report describing the results for the Mill Site Earthworks Project.
