Cripple Creek & Victor Gold Mining Company <br />Phase 5 Slide Repair <br />Final Report ameO <br />Quality Assurance Monitoring &Test Results <br />January 14, 2014 <br />2.0 SUMMARY OF EARTHWORK CONSTRUCTION <br />Earthworks activities associated with the Phase 5 Slide Repair included the following: <br />• SLF preparation /minor repairs <br />• Placement and compaction of backfill in anchor trench <br />• DCF placement above the primary geomembrane layer <br />Ames generally used the following equipment during construction: <br />• John Deere 35D Mini - excavator <br />• Komatsu Excavator <br />• Bobcat T750 Skid -steer Loader <br />• Caterpillar D8R Dozer <br />• Caterpillar D6R Dozer <br />• Caterpillar D6 LGP Dozer <br />• Caterpillar 740 Haul Trucks <br />Weather data for the Phase 5 Slide Repair construction activities from the period of <br />September 2013 through November 2013 are presented in Table 2. <br />2.1 Soil Liner Fill Preparation <br />Ames re- contoured the entire surface of the Phase 5 Slide repair footprint. The <br />surface was smooth, firm, and unyielding; and free of vegetation, construction debris, <br />sticks, sharp rocks, void spaces, ice, abrupt elevation changes, standing water, and <br />cracks larger than one - quarter inch in width. The quality of the finished surface was <br />then maintained until the geomembrane was placed. AMEC and ECA provided formal <br />subgrade (SLF) approval prior to geomembrane deployment. The acceptance forms <br />are presented in Appendix G. <br />2.2 Drain Cover Fill <br />Ames began placing DCF at the Phase 5 Slide Repair in October 2013 and completed <br />placement in November 2013. Ames placed approximately 5,359 cubic yards of DCF <br />per project specifications. The DCF was placed in a minimum of 2- foot -thick lifts over <br />the geomembrane.
