Cripple Creek & Victor Gold Mining Company <br />Phase 5 Slide Repair <br />Final Report ameO <br />Quality Assurance Monitoring &Test Results <br />January 14, 2014 <br />1.0 INTRODUCTION <br />Cripple Creek & Victor Gold Mining Company (CC &V) operates the Cresson Project, <br />located in Teller County, Colorado, near the city of Victor. The Cresson Project is a <br />mining and ore processing and beneficiation facility that is composed of surface mines <br />as well as a valley leach facility (VLF); an adsorption, desorption, and recovery (ADR) <br />plant; and associated infrastructure, all of which were designed and constructed to <br />meet or exceed the requirements established by CRS §34 -32 -101 et seq. and <br />regulations promulgated there under by the Mined Land Reclamation Board (MLRB). <br />The existing Cresson Project was constructed in phases from 1994 through 2011 <br />(Phases 1 through 5). Phases 1 and 2 were permitted in 1994 and constructed during <br />the 1994 through 1996 construction seasons. Phase 3 was permitted in 1999 and <br />construction was initiated the same year, with completion in 2000. Phase 4 was <br />permitted in 2000 and construction was completed in 2004. Phase 5 was permitted in <br />2008 and was completed in 2011. <br />The work associated with the Cresson Project was performed under specific criteria <br />established in Amendment Nos.8 and 9 of Permit No. M- 1980 -244 (Permit), as <br />approved by the MLRB. This Report documents the Construction Quality Assurance <br />(CQA) and Construction Quality Control (CQC) services provided by AMEC from <br />September 2013 through November 2013 for the repair of the liner system at the area <br />of an unexpected slide of Drain Cover Fill (DCF) material. The site location for the <br />Phase 5 Liner Repair is presented on Figure 1. <br />1.1 Project Description <br />The goal of the project was to repair damage to the primary geomembrane liner <br />caused by a slide that caused damage to the Phase 5 geomembrane liner. A leak <br />detection survey of the area was performed in April 2012 by Leak Location Services, <br />Inc. to delineate the extent of the damaged area. Ames Construction, Inc. removed <br />the existing DCF from the damaged liner and ECApplications, Inc. (ECA), the liner <br />subcontractor to Ames, was responsible for removing the damaged liner and installing <br />a new 80 -mil double -sided textured LLDPE geomembrane liner in the Phase 5 slide <br />area, and inspecting the existing soil liner fill (SLF) for damage and make any <br />necessary repairs. Ames then placed DCF over the newly installed liner while taking <br />precautions to not cause any damage to the existing intact liner. AMEC was present <br />to perform CQA monitoring and testing for SLF repair, the placement of the <br />geomembrane, observation of the backfill at the anchor trench, placement of the DCF, <br />and certification of the finished liner system. <br />1.2 Construction Quality Assurance /Construction Quality Control <br />AMEC provided CQA and CQC monitoring and testing services, and prepared this <br />report describing the monitoring and test results for the Phase 5 Slide Repair.
