Cripple Creek & Victor Gold Mining Company <br />Phase 5 Slide Repair <br />Final Report <br />Quality Assurance Monitoring & Test Results <br />January 14, 2014 <br />ameO <br />drawings and supporting CQA monitoring and testing documentation are presented in <br />appendices attached to this report. <br />1.2.1 CQA Monitoring of Earthworks <br />AMEC provided CQC testing and CQA testing and monitoring of earthworks for the <br />Phase 5 Slide Repair. CQA testing and monitoring of earthworks was provided for in <br />place materials in accordance with the AMEC Earthworks CQA Plan (Section 1400.1 <br />of the Specifications located in Appendix E). The DCF material specifications for this <br />project are summarized in Table 1. CQC earthworks testing was provided to support <br />the contractor during the manufacture of materials and prior to material placement. <br />The CQC test results are not included in this report as these tests were intended to <br />assist the contractor with materials preparation rather than record testing. All CQA <br />testing results are included. <br />AMEC provided CQA laboratory and field testing, and monitored earthworks <br />construction activities as follows: <br />• AMEC visually monitored the SLF repairs to confirm the surface was unyielding, <br />free of organic matter or other unsuitable materials, and a continuous soil liner was <br />maintained. <br />• AMEC visually monitored the placement of DCF above the geomembrane in the <br />Phase 5 slide repair area, including verification of lift thickness and grade control; <br />identification and removal of deleterious materials; and verification that care was <br />exercised during fill placement to avoid geomembrane damage. AMEC performed <br />CQA laboratory testing of grain size and Atterberg limits. <br />• AMEC monitored the ambient air temperatures and weather in accordance with <br />project specifications. The ambient air and weather observations during fill <br />activities are summarized in Table 2. <br />1.2.2 CQA/CQC Monitoring of Geomembrane Installation <br />AMEC provided CQA monitoring of geomembrane for the Phase 5 Slide Repair which <br />consisted of performing geomembrane inventory and checking quality control <br />certifications prior to geosynthetics deployment. AMEC performed geosynthetics CQA <br />activities in accordance with the Geosynthetics CQA Plan (Section 1400.2 of the <br />Specifications located in Appendix E). Geosynthetics CQA activities included: <br />• Observation and documentation of geomembrane deployment, trial seams, <br />seaming, destructive testing, and non - destructive testing (pressure tests and <br />vacuum tests) <br />• Third -party conformance testing of geomembrane <br />sAprojects \1568 cc &v - ph v slide repair cqa \11.0 certification report\cc &v roc report.docx 2 <br />
