top of cement was tagged. The drill out of cement plug number 1 began on July 9, 2013 at <br />16:40 hours. The shaker was bypassed to preserve some LCM in the mud for drilling ahead <br />through the remainder of the L5 member. Capstar began drilling new formation at 17:15 <br />hours and drilled 15 feet to 1,955 feet MD KB in the L5 stratigraphic zone of the Parachute <br />Creek Member. At this depth, circulation was lost again. <br />Halliburton was called to spot a second cement plug. The cement crew arrived onsite at <br />23:45 hours on July 9, 2013. They rigged up and pressure tested lines prior to pumping 10 <br />barrels of 10% CaCI water, a 10 barrel fresh water spacer, 10 barrels of SuperFlush, a 5 <br />barrel fresh water spacer, 8 barrels of PlugCem ( 25 sacks, 8.49 gal /sack water, 13.5 ppg, <br />1.79 cubic feet per sack). This mixture was displaced with 26 barrels of fresh water and <br />the drill pipe was pulled back to 1,690 feet MD KB. Sixty -five barrels of fluid was pumped <br />down the annulus with no returns to surface. The cement job ended with the fluid level <br />still dropping and the well unable to hold fluid. <br />NSI stakeholders were advised of the situation and Cement Plug No. 3 was planned. <br />Capstar tripped out of the hole and laid down the HWDP. The pipe was tripped back in the <br />hole open -ended to 1,920 feet to pump the third cement plug. The mixing and pumping <br />sequence for the third plug began at 16:20 hours on July 10, 2013, and was as follows: 10 <br />barrels of 10% CaCI water, 10 barrel fresh water spacer, 10 barrels of SuperFlush spacer, a <br />7 barrel fresh water spacer, and 8 barrels of 13.5 ppg PlugCem mixed with 8 pounds of <br />TuffFiber. This was displaced with 26 bbls of fresh water and allowed to set up for 2 hours; <br />then another 10 to15 barrels of water was pumped down the annulus in an attempt to <br />squeeze some cement into formation. After waiting 10 hours, more fluid was pumped <br />downhole with no returns to surface. It was decided to spot a fourth cement plug. The <br />geologist reported that the losses occurred in the lower portion of the L5 stratigraphic <br />zone. <br />Halliburton was onsite at 03:00 hours on July 11, 2013, and rigged up. The lines were <br />pressure tested to 5,000 psi, and Cement Plug No. 4 was mixed and pumped in the <br />Daub & Associates, Inc. Page 6 of 13 10H -1V Well Summary Report <br />
