(There is no accompanying <br />application for a Stormwater permit in the file.) <br />Please revise the wording of Section 2.03.10 to reflect Oakridge Is intention (or not) to <br />apply for a Stormwater (Construction) permit to replace discharge permit COG - <br />850042. (If issuance of a replacement Stormwater permit is not sought, the Division <br />will conclude that Oakridge intends to maintain the discharge permit until final bond <br />release is achieved.) <br />3. DBMS 04/19: Section 2.03.10 of the abridged permit document lists one water well <br />permit (043612 -F) issued by the DWR, and indicates that this permit was terminated on <br />3/17/82. This information appears to be incomplete, and is not consistent with data <br />compiled by the Division and reviewed by the DWR. Please refer to the enclosed letters <br />referenced in the introductory paragraphs of this letter and ensure that this section <br />accurately reflects the status of well permit(s) issued to Pueblo Coal Co. and/or Oakridge <br />Energy, Inc. for the Carbon Junction Mine. (Related discussion is provided in 2.05.4, <br />below.) <br />DBMS 12/18: The date was annotated to indicate that it reflects the date of application <br />to DWR. However, none of the remaining four wells were added to the section (see also <br />2.05.4, below). Please revise Section 2.03.10 to address all active well permits issued <br />for the Carbon Junction Mine. <br />4. DRMS 04/19: Page 3 -9 of the APD includes an obsolete address for the County Clerk's <br />office. Please revise 2.03.11 to refer instead to the office's current address of 98 Everett <br />Street, Suite C, Durango, CO 81303. <br />DRMS 12/18; The address was updated; item is resolved. <br />DBMS 04/19: The Public Notice provided on pages 3 -10 and 3 -11 of the APD was <br />revised prior to completeness and publication. DRMS 12/18: In subsection 2.03.3 of the APD, the office of Greg Lewicki and <br />Associates is not listed, although certain of the permit maps and text were prepared or <br />modified by this firm during the TR -11 process, Please add Lewicki to the list, if <br />appropriate. <br />7. DRMS 12/18: In subsection 2.03.4 of the APD, Bryan Duff, P.E. is listed as the resident <br />agent for Oakridge. The Division has become aware that Mr. Duff has retired from Goff <br />
