Savage & Savage 18- Dec -2013 <br />C- 1992 -080 / RN-4 PAR No. 2 Page 2 of 7 <br />2.03 —Legal, Financial, Compliance and Related Information <br />L ARMS 04/19: The RN -04 application form provided updated contact information for the <br />ownership and control of Oakridge Energy, Inc. The Division conducted a review of the <br />Applicant Violator System (AVS) on 29- Mar -2013, and found that the application is not <br />yet adequate. Specifically, the ownership and control is not consistent with the <br />information in the AVS. AVS has Carol Cooper listed as a Corporate Officer (start date <br />111/98), Danny Crocker as Assistant Secretary, Treasurer and Vice President (start date <br />511192), Sandra Pautsky as Chairman of the Board (start date 719198), Sandra Pautsky as <br />Director (start date 8/10192) and Sandra Pautsky as President (start date 6125198). Please <br />provide end dates for the people listed above, if they are no longer Officers / Directors. <br />DRMS 12118: A letter from Oakridge Energy, Inc., dated 29- Apr -2013, was provided <br />with the 30 August response. End dates for Carol Cooper, Danny Crocker and Sandra <br />Pautsky were provided in the letter. The Division is currently checking the AVS; we <br />will inform Oakridge is any additional action is required. <br />2. DBMS 04119: Section 2.03.10 of the APD indicates that the discharge permit (COG - <br />850027) issued by the Water Quality Control Division has been terminated. <br />Documentation of the permit's termination has not been submitted to the DRMS, and <br />according to correspondence from the WQCD (as documented in the March 26, 2013 <br />Inspection Report), that agency requested that the mine be covered by a Stormwater <br />permit until final bond release has been achieved. Please submit documentation <br />confirming that the CDPS permit has been terminated, and that a Stormwater permit has <br />been issued for the Carbon Junction Mine. <br />DRMS 12/18: Section 2.03. 10 of the APD was revised to indicate that termination of the <br />discharge permit is in process. The cover letter states that there has been no response <br />from the WQCD regarding the application. Correspondence between Oakridge and the <br />WQCD regarding this issue is on file with the permit documents at Goff Engineering; it <br />appears that the WQCD may be awaiting action from Oakridge.
