Mine Site Inspection <br />Mine Name: Williams Fork (Eagle 5 &9) Mine <br />Permittee: Moffat County Mining, LLC <br />Permit ID #: C- 1981 -044 <br />Inspection Date: November 19, 2013 10:45 — 5:00; November 20, 12:30 — 3:00 <br />Inspection Type: Complete Oversight <br />Weather Conditions: Overcast, cold (30's — low 40's) and rainy on Tuesday, partly cloudy, <br />cool (40's — 50's), wet ground conditions on Wednesday <br />Participants: <br />Christine Belka, Office of Surface Mining Reclamation and Enforcement (OSM) #182 <br />Jared Ebert, Colorado Division of Reclamation, Mining, and Safety (DRMS) <br />Spencer Shumate, OSM <br />Brian Watterson, Moffat County Mining, LLC (MCM) <br />This was a complete oversight inspection. Williams Fork (a.k.a. Eagle 5 &9) is a nonproducing <br />surface / underground mine complex that is currently in Temporary Cessation (TC). Due to its <br />TC status, DRMS has instituted a reduced inspection frequency of four (4) complete inspections <br />per year. <br />Backfilling and Grading (Rule 4.14): Because this mine is in TC, the operator is not actively <br />reclaiming the site. However, the reclamation that has occurred in the past was visually evaluated <br />during this inspection. The operator is considering dismantling the stack tube and conveyor <br />system because the necessary width of conveyor belt is no longer commercially available, so the <br />system would need to be replaced if the mine went back into production. <br />Portal 5 is sealed, stable, and well vegetated. Portal 5A is sealed and stable. The old drill pad <br />area has been seeded, but vegetation is still sparse. MCM plans to drill seed this area in the <br />spring. <br />The Williams Fork strip pit location is backfilled, graded, and well - vegetated with herbaceous <br />and woody species. No erosive features were noted. <br />Roads (Rule 4.03): All roads traveled were generally in good repair. The road leading to the Utah <br />Tract was not passable due to muddy conditions. Such conditions are usual for this area in winter <br />and do not indicate a problem with road maintenance or drainage. <br />Coal Mine Waste (Rules 4.10 and 4.11): The waste disposal site is approximately level with the <br />surrounding topography. Waste is contained in the designated area with no evidence of sediment <br />leaving the site although vegetation is sparse in some areas. <br />Hydrology (Rule 4.05): Diversion 5 -D2 is stable and well vegetated through its entirety, but has <br />a constructed berm in the southern segment. The land to the west appears to have been used as a <br />borrow area to construct a berm bridge across the diversion channel. No culverts were placed to <br />allow flow under the berm (see photo 13930). This may be remedied by rerouting the flow <br />
