The vegetation around the
waste disposal site was sparse in areas and thicker in others. No problems were noted.
HYDROLOGIC BALANCE -Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage —Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
The drainage channels (Diversion Ditches, Collection Ditches and Mine Water Ditches) located at the site
were observed. The drainages appear to be in the locations depicted on Map 26 from the permit. Most of the
drainages are heavily vegetated with grass. Some drainages had debris such as trash and branches that will need to
be removed.
Diversion ditch 5-D2 was walked. Near the southern end of this diversion ditch, a berm has been constructed
across the ditch. This berm will need to be removed and the ditch reestablished. Or, if this berm serves a purpose,
the Operator may install a culvert under the berm to allow the water from the ditch to bypass the berm. If the latter
option is chosen,the Operator will need to submit a Technical Revision to the permit to update the sediment and
drainage control plan with designs for this culvert.
Pond 5-P3 is a shallow pond and was not holding water.
Pond 5-P2 was not holding water or discharging. The embankment was stable and vegetated and the discharge
point was clear and functional.
Ponds 5-P5, 5-P6 and 5-P7 were observed. These ponds were not holding water and not discharging. The
embankments were stable and vegetated. The flume was partially obstructed with rocks and will need to be
cleared out.
Ponds HR-Pla and HR-Plb were observed. These ponds were not holding water and not discharging. The
embankments were stable and vegetated. The discharge points were functional.
Ponds 9-P7 and 9-P8,were not holding water and not discharging. The side slopes of the ponds were stable
Pond 9-P6 was not holding water and not discharging. The embankment was stable and vegetated. The spillway
was clear and functional.
Pond 9-P 1 was not holding water and not discharging. The embankment was stable and vegetated. The spillway
was clear and functional. Some minor pond slope erosion was observed at the north east portion of the pond.
Pond 9-P5 was not holding water and not discharging. The side slopes were stable and vegetated.
Number of Partial Inspection this Fiscal Year: 0
Number of Complete Inspections this Fiscal Year: 2
Page 3 of 7
